Employee was getting into truck when he fell backwards onto rocks, hitting head, although he had his hard hat on.

While descending the steps of the loader, Injury hit his head on the metal support bar for the left side mirror. The mirror had been removed for repairs leaving a bare metal support bar protruding out from the top of the loader.
While grinding a shaft on the oversize conveyor, a particle flew into EE's right eye.
While installing a pin into an excavator, EE missed the pin with his hammer and hit the pin with his hand instead. The impact cut his middle finger on his right hand and required stitches.

While using a chipping hammer inside a cone crusher, a chunk of material broke loose and bruised his left foot.
While getting out of the maintainer EE accidentally hit the controls which caused the blade to push further into the ground. When this happened, he lost his balance and fell, landing on his right leg.
Employee swung a sledge hammer and felt a pop in his right bicept while installing a pin into the bucket of the 325 longstick.

Driver had clot of dirt caught in the tail gate of his end dump trailer, he and the witness lifted the tail gate high enough for the dirt to fall out and the tail gate came down and caught the driver's hand between the bed of the trailer and the tail gate.
While laying on back, Employee experienced difficulty adjusting breaks. He tried to force the nut and strained his left side and back.
He missed second step while descending from the cab of his truck. This caused him to fall on his left foot and twist/sprain his left ankle.
Climbing small ladder on side of portable plant, had mud on boots, leg slipped inside the ladder and he fell backwards hitting his head on the ladder, upside down.
Removing rock from between tires with a piece of leap spring. While removing the rock, the tire exploded sending rock fragments in several directions. A piece of rock struck the victim on the right hand, cutting through the gloves and causing lacerations.

EE CLIMBED ON TOP OF AN AREA HE SHOULD NOT HAVE. OTHERS NOT AWARE OF HIM ON BELT STARTED MACHINE. EE STILL ON LIGHT DUTY, PHYSICAL THERAPY FOR KNEE.
PERSON WORKING ALONE TO REMOVE BATTERIES FROM 200 KOMATSU (WEIGHING APPR. 100 LBS APIECE). REQUESTED ASSISTANCE AFTER LEANING DOWN TO REMOVE, BATTERY SLIPPED, HE GRABBED IT INJURING HIMSELF.
IMPROPER LIFTING BY EE & THEN CONTINUING TO CARRY ITEM ABOVE CONTRIBUTED TO MUSCLE SPRAIN - STRAIN IN BACK.
HE WAS DESCENDING LADDER ON BACK OF SERVICE TRUCK AND HIS FOOT SLIPPED ON THE LAST STEP. HE FELL BACKWARDS ONTO THE TANKS AND THEN UNTO THE BED OF THE TRUCK. IMPROPERLY DESCENDING ON THE LADDE R AND UNCLEAN LADDER SURFACE CONTRIBUTED TO FALL.
