Fall from ladders
EMPLOYEE WAS PRYING ON A BOULDER WHILE STANDING ON A LADDER. THE PRY BAR CAME LOOSE AND HE LOST HIS FOOTING AND FELL APPROXIMATELY 4 FEET LANDING ON A ROCK.
