Over-exertion (Not Elsewhere Classified)

Climbing up to put oil in the water scraper, EE's left foot got caught on the lip at the top. EE stepped up with EE's left foot and was bringing up EE's right foot and EE's left foot slipped. EE had a hold of the hand rail and tried to catch self, EE's right foot got caught on something causing the injury.

EE finished greasing the crusher and was climbing down and didn't notice the cross bar and stuck EE's hand thru it and as EE was stepping down EE's index and middle finger on EE's right hand were pinched.

Caught in, under or between running or meshing objects

Employee was loading a truck with 200 lbs rip rap and one boulder fell off and they didn't notice it. They pulled forward running over the top of the boulder, that caused them to jar their back. Went back to work but as the day progressed so did the pain in their lower back and they notified their supervisor.
