Employee was closing the entrance gate to mine and slipped on snow/ice. Employee strained upper leg(thigh.)

Was servicing piece of equipment when the radiator door strut on the skid loader failed and came down on left forearm, with some velocity.

Rubbed or abraded

Miner was attempting to start a pull start pressure washer, when he pulled on the rope and released the rope his left index finger got caught in the rope and pulled the skin back between the last and middle knuckle.

Fall onto or against objects

Individual was climbing out of the water truck to turn off the PTO pump and tripped on the fuel cap causing him to hit his knee on the ladder of the truck.

Was working on conveyor belt cutting the conveyor belt when the knife slipped cutting right palm of hand.

Contact with heat

The miner was working outside in the heat. He entered the scale house complaining of being hot. He drank some water and complained of nausea and said he had stopped sweating. He was taken to K-stat to be checked out.
