He was on the catwalk of the Masaba plant changing out a screening plant. While he was carrying the screening plant his arm twisted in an awkward position due to the plant slipping from him, and he felt a popping occur in his shoulder.

Struck by flying object

While crawling into a screening plant, a piece of rust fell off the screen into his eye.

Contact with heat

Employee started throwing up, had numbing in the face, stopped sweating, had dizziness, and was disoriented due to extreme heat. Employee did not lose consciousness and was coherent at all times.
