Struck by falling object

Was unhooking a chain on a conveyor. Thought the conveyor was on the ground but was hung up on something. The conveyor released from whatever was keeping it up and dropped on right foot.

Bodily reaction, (Not Elsewhere Classified)

Was working on a scalping screen removing a bolt with right hand. Employee turned with shoulders to hold the bolt with the left hand when their back popped.

Over-exertion (Not Elsewhere Classified)

Had a bar stuffed up in the bin trying to dislodge a boulder. The bar slipped and ee jammed left shoulder.

Struck against stationary object

Was loading a screen into the loader bucket, was not wearing gloves and got finger caught on the edge of the screen cutting right index finger.

Fall onto or against objects

Was walking to a skid loader when ee tripped and stumbled, tried to catch self and fell into the spoon attachment on the skid loader.

Rubbed or abraded

Was installing a stick cylinder on the 600 excavator,CS60. Was removing the lifting strap from around the cylinder, laid left arm on steel hydraulic line so they could use both hands. When they stood back up drug their arm across a burr on that line, slicing open the left arm from the wrist to the forearm.

Struck by falling object

EE was helping a co-worker remove a guard from Q-173 portable crusher and the guard slipped and fell hitting EE's right shoulder.

Inhalation of radiations, caustics, toxic and noxious substances

Was operating a haul truck with an exhaust leak for a day and a half before EE realized what was going on. EE was put in a different truck and the mechanic was called to repair the truck. The accumulative affects hit EE on Thursday on the way into work and EE went to the emergency room.
