A roof fall occurred in the gob area behind the longwall face of Gil 4 MMU 010-0 disrupting ventilation. Ventilation devices in the South Mains and Giil 4B cross over were damaged.

Roof fall occured in the 2 North Mains at crosscut 4 in the return air-course.
Carbon monoxide samples in the recently opened West Lease, North/South entries began to demonstrate increasing and fluctuating values to a point that we determined to call MSHA. Sufco had been consulting with the price filed office concerning the sample results. There was not a sudden influx or inundation of gases.
Miner went from managing a pump to aid dewatering at the face transferring slop from 5 gallon bucket into a tote. When dumping bucket, miner felt a ""pop"" and experienced pain in the back. Miner was brought out of heading and was taken to the hospital.
Operator was cutting a crosscut. As mining advanced, operator stepped away from rib mesh to look at cutting. At this point a rib fell and knocked operator to the ground. Operator received two broken ribs, cuts to the face, and a pulled shoulder.
While operating Hilti Fastener Gun, employee placed his hand over barrel and gun fired.
EE was installing a primary support when a rock hit the ground and rolled into his left foot and ankle.
Employee was installing the primary roof support with a jack leg drill. The air hose came loose striking him in the right hand.
While collaring a bolt hole with a jackleg drill, a slab of rock approx. 12"" x 12"" x 4"" fell from the top of the rib and struck the miner on the back of his left hand, cutting through his glove and causing him to sustain a laceration on the back of his hand resulting in six stitches.
EE was replacing a nozzle on a wash hose. Piece of coal (20""x8""x3""thick) slid down the rib and struck right hand. EE moved the wash hose valve next to the rib which was protected with wire mesh within 3' of the floor.EE was earing keviar gloves. Same knuckle was fractured of the job 3 months prior resulting in this re-fractured. No external marking or bruising of skin.
