Accident type, without injuries

A roof fall occurred in the return escapeway of 2RWest Lease continuous miner section near the overcast near x-cut #2. The fall was 10'x 15'x7' high. 3-7' roof bolts fell when the top fell. MSHA notified at approx. 8:30 a.m. MSHA insp. were on-site when the fall was discovered and were aware of the fall prior to hotline notification.

Accident type, without injuries

At approximately 11:55 a.m. Sufco's tracking system lost communication in the bleeder area of 2 South. Tracking system power supplies in the area were checked first and found to have power. Two employees were dispatched and found a roof fall at #30 x-cut in #2 entry of the 2 South bleeder. All entrances to the area were immediately dangered-off and MSHA notifies. 8'Hx18'Wx18'L.

Over-exertion in pulling or pushing objects

Crew was pulling disco pipe into beltline with loader. Connection anchoring pipe to loader broke and struck injured.

Caught in, under or between a moving and a stationary object

Carrying a crib block and slipped on ice and fell, the crib block landed on his hand.

Fall to the walkway or working surface

He was walking on the surface, in the snow, when he slipped and fell. He fractured his right Fibula.

The mine was rock dusting when the employee went down to pre-shift. He was looking at the Roof and Ribs for hazards. Due to the rock dust being in the air, visibility was poor. Employee lost his footing and slipped and fell and strained his knee.

Employee was cutting a cardboard box with a knife when the knife slipped and cut his leg.

Struck by falling object

IE was attempting to free a bundle of roof mats that were being loaded onto a roof bolter. The mats became stuck and IE attempted to free stuck bundle. A banding strap on the bundle of mats broke in this process and a mat fell from the bundle striking IE on the right leg resulting in a laceration which required sutures.
