Longwall downshift crew was moving power. While moving the power center out of crosscut 14, several rock props were inadvertently knocked out. Before crew could reset rock props, the roof took weight and fell. Area that fell was approximately 20 feet wide, 20 feet long, and maximum of 4 foot high. This area during advancement had been bolted with 7 foot rebar, resin grouted bolts.

Coal and rock roof approximately 33 feet long by 10 feet wide and up to 7 feet high fell on and around the long-wall stage-loader.
Injured employee was helping install roof bolts in a coal face during longwall recovery operations. His left index finger became pinched between the head of a roof bolt and a bearing plate causing the finger to be severed at the end near the first knuckle.
Injured was turning off a fire line hydrant during a fire drill when the fire hose failed and water pressure pushed him into belt structure. Ligament damage to left knee.
Injured was turning off the water sprays on longwall shearer when a rush of air pushed him down causing him to strike his knee on a shield toe. He was taken to an emergency room where x-rays proved negative.
Injured employee was attempting to get a D-ring on a shield free of mud and struck his finger between a steel pin and the shield base. Fractured right index finger.
A medical examination of MIIN M82797435 indicated that this miner had a sufficient degree of pneumoconiosis to be eligible for Part 90 rights. M82797435 exercised the option to work in a low dust area which has been complied with since said date. This report is being filled out to comply with Part 50.20(a) and to abate citation #9026945.
