Accident type, without injuries

Power tripped to one of two longwall power centers in 2R6W section. Power dropped to entire section. Smoke and heat were detected at the power center, all miners were removed to outby the power center, and fire fighting efforts were initiated. Ventilation was re-routed and a fire was extinguished within the power center within 45 minutes of discovery.

Accident type, without injuries

Roof fall on the longwall stageloader. This fall was 12' X 18' at the widest point. It separated at 7 1/2 foot level and laid down on the stageloader. It did not impede passage or block escapeway. MSHA was called and a plan was submitted and approved.

Caught in, under or between a moving and a stationary object

After rock dusting activities, employee was placing the hose into the bucket of the scoop. At this point, the bucket was inadvertently lowered onto the injured's right foot. The injured was taken to the hospital where x-rays revealed no broken bones. EE was prescribed days off work.

Struck by powered moving object

Employee was firebossing and adjusting the air on section and a shuttle car bumped into the employee and the employee fell against the rib and felt pain in back and legs.

EE reached for the next crib to move as another EE went to put a crib down on top of stack. Crib block landed on top of EE's hand.

Employee was using a pointed bar to operate a track jack. Employee stepped on the point with right foot to push down. The point went through EE's boot and broke the skin.

Struck by rolling or sliding object

After cutting through the headgate, the shearer began heading to tailgate. When the headgate drum was at shield 7, the injured miner stepped from shield 3 to shield 4. The upper part of the face wall came in, filled up the pan and knocked injured against shield leg ram.

Injured was hit by rib slough and fractured the collar bone.

Over-exertion in pulling or pushing objects

EE was pulling on a line of ventilation tubing when they felt a pop in the left bicep. This resulted in a ruptured bicep tendon.

Loader operator dropped bucket on Employee's boot while Employee was loading a vent tube in bucket.

EE removing a roller on belt line when left ring finger was caught between the end of the roller and the roller frame causing an avulsion injury to the tip of the finger.

While working on shuttle car conveyor, employee was struck on the hand by a hammer

Employee was assisting with a belt splice and got hand hit with a hammer causing injury.

GMS employee was carrying a drill and stepped on a piece of coal and rolled ankle causing injury.

While stepping off loader, employee twisted ankle.

Fall to the walkway or working surface

While walking EE slipped on a small patch of ice that had a light coating of snow. EE fell, landing on their right arm. EE had pain in right shoulder.

Fall from ladders

EE was tying roof mesh to existing roof supports. EE was standing on a ladder in a x-cut by the haulageway. The piece of mesh EE was hanging hung down on one edge and was struck by an on-coming truck which pushed the mesh into the ladder, tipping the ladder over and EE fell to the ground, landing on right arm.

Over-exertion in lifting objects

EE was preshifting a Dodge pickup. EE attempted to lift the engine compartment hood with left hand and the hood wouldn't move. EE then jerked on the hood with left arm and the hood still wouldn't move. EE felt pain in the left arm. Overexertion/strain to left bicep tendon.

While removing a 90 pound brake from shuttle car, employee's hand was hit and received lacerations.

Contact with electrical current

EE was helping move a de-watering pump and when hooking the insulated hook and cable onto the rib mesh, ee felt an electrical shock. EE finished shift and had pain in right forearm. No evidence of skin damage.

Replacing AFC head sprocket cover, EE had hands on front of cover to align into position, when tension was released cover moved quickly into place. EE's hands rolled forward pinching EE's fingers between cover and frame.

Struck by falling object

Employees were rehanging brattice cloth in relative darkness when an unseen rock fell out of the brattice cloth, striking one employee's right pinky fingertip. Physician drained accumulated blood from contusion.

Over-exertion in lifting objects

Employee pulled groin lifting a box. Employee was seen at the ER and was released to RTW. After a follow up visit with a specialist, it was determined on 08/31/2018 that surgery was needed. Employee has not missed work.

Employee pinched right ring finger when the employee placed a fire hydrant on front end loader.

Struck by flying object

Maintenance was being performed on A 9"" long bolt flew through the air and hit the face.
