Accident type, without injuries

During an escapeway drill, a crew found a location where the roof rock had fell up to 4 foot high in the secondary escapeway.

During the weekly examination, the examiner found a roof fall that was above the bolt anchor zone. This area was not in an escapeway, travelway, or roadway. And, it did not impede travel or affect the ventilation.

Struck by falling object

EE was installing cable bolts using two-piece steel. EE had removed the driver portion of the steel combination and the finishing 6' portion of the steel fell from the hole striking EE's right hand against the drill extension wrench. This resulted in a fractured right ring finger.

EE was helping load roof bolting supplies on the back of a roof bolter. The roof bolter operator was positioning the machine and lowered the back end of the roof bolter on EE's left foot pinching the foot between the machine and the floor.

EE was positioned in between a parked utility vehicle (UV) and a trailer that was attached to the UV. The UV was running and lunged backward when a second employee adjusted the idle of the UV. EE was pinched between the rear bumper of the UV and the tongue of the trailer. Fracture of right leg.

EE was helping move a rock from between the leg rams of a longwall shield. As the rock was being situated and moved forward, EEs left little finger was pinched between the rock and the leg ram.

While putting a metal cover back on roof bolter, employee smashed the tip of EE's index finger

Fall from machine

Two employees checking wear plates on crusher chute in sub-zero temperatures. Inspection doors on chute were frozen shut. EE attempting to open door latches with hammer fell from a kneeling position landing on shoulder and head causing fractures and lacerations.

Over-exertion in lifting objects

Employee lifted a 3"" drisco pipe with shoulder. Employee felt a pop and was taken to hospital. ER Dr. gave employee a return to work release. Employee has not missed work due to this incident. Employee seen a Dr. on their own on 2/17/2017 and had surgery on 5/11/2017. It is now determined that the incident on 12/5/2016 is reportable.

GMS employee was carrying some metal on EE's shoulder. EE set is down and smashed EE's finger against another piece of metal.

Witnesses stated employee was on a ladder grinding. EE came down to the platform and stated they felt ill. EE sat down and then laid down. Witness went to summon assistance. Sufco EMS transported EE to the Richfield area and then met with Sevier County EMS. Then Sevier County EMS took EE to the Hospital, Richfield, Utah.
