Roof fall occurred in the 3RWL longwall bleeder in 3 entry @ 7 x-cut. It is uncertain when the fall occurred as it was 150' behind LW face and no person witnessed the fall. The fall measured approximately 21'x 60'x 14'high. The area out-by was supported with double-row of wood cribs. MPLs for weekly exams of the bleeder have been set and approved by MSHA at x-cut #6 & 11.

A roof fall occurred in the 3RWL #2 entry, #7 x-cut. Seven foot resin bolts initially installed when developed. The fall dimensions were approximately 20'x 20'and 10' high. No persons affected by fall. Electrical and communication cables re-routed. Escapeway re-routed. Re-habilitation work proceeding according to MSHA approved plan using 12'cable bolts.
While shoveling the employee pulled a muscle in lower back. This occurred on 12/12/2017. Employee did not report this incident until 12/19/2017. EE seen a doctor on 12/21/2017 and then at a follow up visit on 01/02/2018 it was determined employee needed days off for physical therapy.
Employee was stepping in to the back of the shield to do repairs. foot slipped off shield and bend backwards. Employee refused to seek medical attention until 1/4/2018. At that time it was discovered employee had a fractured foot.
EE was assisting with the removal of a broken top belt roller. The roller was under tension from the top belt and when it was pulled free of the tension, it rotated upward striking EE on the face in the left cheek area.
EE was ascending stairs at an overcast when they stumbled near the top and fell striking the left upper arm on the top of the handrail. This resulted in pain to the lower bicep area. EE continued to work regular shifts through 11/19/2017.
Employee was pulling manifold line from pumping and felt pain in ee stomach area.
EE was servicing LW shearer when sloughing plate fell striking EE on left shoulder and right forearm. Dislocation to left shoulder, sprain/contusion to right forearm.
EE was servicing a lube truck and while walking around the truck ee stepped on a lump of coal which caused to fall to same level resulting in pain to left hip/leg.
A GMS employee, working on a GMS specialty project at Sufco Mine, was unloading a trailer of ball busters. One broke loose and employee smashed finger between two ball busters. GMS crew leader was witness.
Employee was assisting on the roof bolter. While the right side operator was putting in a rib mesh, injured tried to grab a bundle of plates that had slid under supply tray. At the same time the operator lowered the mast and pinched the employees hand.
Injured miner was assisting with belt repair when finger was struck by a single jack hammer resulting in a fracture to the tip of left index finger.
EE was mounting a cutter motor on a continuous miner. EE lost balance while stepping off on an 18"" high rail causing EE to fall backward. A pry bar EE was using struck EE in the right side as EE fell causing a 1"" laceration.
EE was replacing foot shaft for continuous miner. EE was holding a punch with a punch holder while another miner struck the punch with a hammer. The hammer rebounded off the punch striking EE on the right side of cheek fracturing teeth.
GMS employee was retrieving a bag of rockdust near the rear of the bolting machine and the rear of the bolting machine was lowered causing injury to the employees foot.
Employee was working on a Longwall shield. Employee pulled a staple out of a valve fitting and was struck by the valve in the forehead resulting in a 1 inch laceration.
EE was arranging cover plates on stage loader conveyor using a come-a-long when hook came out of anchor and struck EE on chin creating a small laceration and breaking a front tooth.
While unhooking a 3"" soft water discharge line, the drisco pipe flipped up and hit employee in the teeth.
