Caught in, under or between a moving and a stationary object

EE was operating a drill press in the welding bay to drill holes into a bracket, EE secured it in a vice. While standing to the right, EE held the vice with EE's left hand and operated the press with EE's right. The bit bound, causing the bracket to spin out of the vice which caused EE's left thumb to be caught between the vice and bracket.

Caught in, under or between a moving and a stationary object

EE was pulling cable from a spool at the cable winder. A chain meant to keep the spool centered came loose, allowing it to shift and create a gap near the stops. The EE reached for the knuckle and, while pulling it down, placed their hand into the gap, where it became caught in a pinch point.

Struck by falling object

Drilling with a jack leg drill. EE removed drill pressure and the drill dropped but the drill steel stuck in the roof. EE grabbing the top of the drill to keep it from falling when the drill steel fell from the hole, striking EE's finger. EE was taken to the ER and diagnosed to have sustained a laceration and fracture to the right index finger. It required stitches and a splint.

Struck by flying object

While using a track jack, the jack bar slipped out of the employee's hand and struck them in the chin causing a laceration.
