Employee was attempting to change position of the gate at the final wash plant to change the blend of material. Employee said he felt no pain but later started to have pain in his right elbow on Saturday morning. The incident was reported to his supervisor on 01/07/2008 and medical treatment requested. The employee was put off work on 01/29/2008 and referred to an orthopedic.

Employee was installing a lid on a new material tank, wind was blowing, material from job site blew into employee's eye.
Plant lead man was patching an outer jacket on the trommel screen. He was using a bolt like a hammer to punch a bolt through a hole in the patch. He missed the bolt on this particular swing and struck his left hand injuring his ring and pinky fingers. He suffered lacerations to both fingers requiring 10 stitches and also dislodged his pinky nail.
Employee was grinding on horizontal angle iron. As he was looking down while grinding, a piece of metal hit an upright angle iron and bounced back over the top of his safety glasses causing small pieces of metal to enter his left eye.
EE and co-workers were unloading duct work off the boom truck. When the duct was lifted, it swung off the truck and pinned the EE between the duct and a golf cart parked in the area bruising EE's left leg.
EE alleged that he was lifting an 80 lb gas bottle when he felt a sharp pain in his lower back.
EE was working in the Raw mill with over head cutting taking place. EE went to put on a pair of gloves to remove some metal from the area that had been cut away from the job taking place over head. When he put on the right hand glove he felt something like he had been burnt by a hot ember. But it was a brown recluse spider in his glove that had bitten him.
Employee was repairing lights on Utility trailer ( Pole ). While cutting cable ties that holds wiring harness, knife lacerated his 5th/ little finger on his left hand, resulting in finger receiving 3 stitches.
EE was washing the calciner and a piece of buildup fell on the end of the lance that was being using raising the back of the lance up and striking the EE under the chin causing mouth to slam shut and jamming teeth together. Chipping his front top tooth.
Utility knife being used to cut conveyor belting slipped causing a small laceration to the left leg requiring two stitches.
Employee was loading shot and had a non fatal heart attack.
