Struck by falling object

The employee was attempting to replace a return roller. EE laid the roller on two pieces of angle iron and was resting hand on the conveyor frame while EE waited on someone to come help. For some reason the roller fell off the angle iron and struck EE's hand. This caused an amputation to the distal phalanx on right ring finger.

The employee had a piece of slag fall into ear while welding. The employee reported it to manager, but did not seek medical attention at that time. The employee later saw a physician and was advised the injury required surgery, which occurred on 5/9/17. We are reporting this injury within 10 days of the date of surgery.

Injured employee was assisting service personnel on a dewatering pump. Subject slipped and cut lower right leg on unknown object.

The contract employee was removing drill steel when the pipe wrench made contact with the contract employees thigh causing a laceration. The work that was being done by the drilling contractor was being done for Dade County and was not mining related.

Fall onto or against objects

The employee was attempting to tie a rope around a tree to stabilize the dredge for routine maintenance when employee lost footing and caught forearm on a jagged branch causing a laceration requiring stitches.

Fall onto or against objects

The employee was attempting to get up from EE's sit in the operator's booth of the dredge when EE's coat was caught on the chair causing EE to lose balance and strike head on the door frame.
