Employee was putting tipping valve weight back on tipping valve arm when shaft rotated to the up position causing weight to slide down shaft arm and pinch employees left ring finger between weight and valve body. (Employee was suspended for remainder of work schedule upon further investigation Employee was terminated for violation of Lock out/Tag out policy.)

Struck by falling object

WHILE GRINDING, A SMALL SPEC OF METAL FELL INTO EMPLOYEE'S RIGHT EYE. AFTER SEEN BY A PHYSICIAN THE METAL WAS REMOVED. EMPLOYEE WAS GIVEN EYE DROPS AND RETURNED TO WORK THE NEXT DAY (HIS SHIFT ENDED AT 1530 WHICH EXCEEDED THE DR'S VISIT). HE WAS WEARING A FACE SHIELD AND SAFETY GLASSES AT THE TIME OF THIS EVENT.

Struck against a moving object

For unknown reasons, the customer dump truck gradually overtraveled the berm without any apparent correctible or braking action and rolled upside down into a water-filled canal bordering the property. The driver is an employee of Al's Excavating (a/k/a JR Trucking)
