Fall to the walkway or working surface

EE slipped & fell on buttocks while boarding work boat. EE refused medical treatment until 7/8/10 when he requested & was sent for evaluation. Upon evaluation, EE was released to duty in his assigned job with no limitations. On 4/26/11 EE returned to Dr & was placed on modified duty. We cannot accommodate medical restrictions & have placed the EE on LT status on 4/27/11.

Fall from ladders

Employee working from step ladder fell approximately 3.5 feet to the ground, landing on his right leg hurting his ankle. 911 was contacted, ambulance responded, and transported to local hospital. Doctors at hospital evaluated his injury and determined he had a broken ankle. Employee is currently off duty until doctor's release him to return his job.

Struck against a moving object

Employee was operating front end loader while digging in the face of a sand pit. While digging, the loader struck a large rock causing the employee's head to lurch forward. The employee did not seek medical care until 10/18/10. He was diagnosed with a neck strain and put on restricted duty. He was returned to full duty on 10/25/10.

Employee slipped and fell landing on hand, catwalk grading caused laceration on right ring finger.

Caught in, under or between a moving and a stationary object

EE was trying to close bottom hopper door on coal car, using a 20 ton hydraulic jack to close last few inches to pin in place. Bottle jack slipped, causing door to spring open and pinch EE finger between the door and grizzley grating.

EE left hand was caught between dump hopper and forklift mast after dump hopper was sat on ground EE attempted to push hopper of forks, hopper tilted backwards pinching EE palm and thumb.

Fall from machine

While dismounting front end loader after servicing the grease tank, the employee'As left foot slipped off the third step causing his left hand to slip off the railing and lose his balance, which resulted in the fall to the ground of approximately 3 feet. The fall resulted with fairly minor injuries. Dr. prescribed pain medication which triggered 7000-1 reporting.
