Struck by falling object

OVERHEAD ROCK, ROCK FELL AND STRUCK RIGHT HAND.

Caught in, under or between running or meshing objects

A large rock lodged in conveyor at head pulley, employee got on belt and kick belt rock out, when EE did the rock went out and EE's leg was trapped between the belt and head pulley, EMS showed up and cut the belt to get EE's leg out, the counterweight dropped and through them off and they were able to get EE's leg out.

Caught in, under or between a moving and a stationary object

Employee was using a drill to tighten the nuts on the lower side of the apron of a crusher when the load moved sideways to the ground catching the employee's finger causing a laceration and fracture.

Struck against a moving object

Employee was driving the forklift, hit a bump, bounced in the seat, and struck left testicle between employee's leg and the seat.

Employee was holding a pipe with left hand and a 4 1/2 angle grinder with right hand when the grinder kicked back striking the employee's left thumb.

Contact with heat

EE was working outside and stated didn't feel well.
