DROVE VOLVO THROUGH PLANT 7 WITH BED UP. STRUCK POWER LINE & TORE POWER LINE DOWN. TAKEN TO COMMUNITY HOSPITAL TO BE CHECKED. NO INJURY, WAS DRUG TESTED AS WELL. *EMPLOYEE TERMINATED*

While repairing and cleaning inside of 9A Ramsey Feeder employee stopped the feeder to examine it, thought the feeder had stopped turning then put his right hand into the paddle wheel of feeder, but it was still coasting to a stop which resulted in severe cuts to all fingers on employee's right hand.

Walking & stepped on a rock and fell back, leg bent back, hurt knee.

THIS ACCIDENT OCCURRED WHEN A LARGE PNEUMATIC WINCH BROKE FREE FROM ITS ANCHOR. THE WINCH PINNED THE INJURED EMPLOYEE AGAINST AN I-BEAM STRUCTURE. THE ACCIDENT CAUSED CRUSHING INJURIES TO HIS LOWER TORSO (CRACKED PELVIS, TORN BLADDER).

Block moved unexpectedly while greasing. Caused bruises to upper right side of body and left knee

Employee was moving spare parts at the plant. He was engaging a chain through materials to be moved. One part shifted position and his hand was caught between the two parts.

Struck by flying object

He mechanic was working on a brake cylinder which was mounted on truck. Started to tighten bolts and and bolts stripped from studs causing chamber to separate.

Employee was checking rock shaker to see if it was locked down. The counterweight was stopped on the topside, and when the employee moved the sheave, the counterweight rolled to the bottom, catching his hand between the belt and the sheave. This caused lacerations to the right index and second fingers.

Employee notified us of accident on 2/21/05. Employee was standing in scale house; claims spider bite on the right inside leg.

Was removing mill bearing on Cedar Rapids Crusher. He was using a piece of metal round stock to loosen bearing race from mill bearing housing. With continued blows of the round stock to the bearing race he has developed acute tendinitis in his right elbow.

Was standing behind a truck rolling hose, lost footing, slipped forward, tried to catch the back of the truck, back began to hurt.

Struck by falling object

WHILE ASSISTING MAINTENANCE PERSONEL IN ADJUSTING A GEAR BOX, EE PLACED HANDS UNDER THE HOUSING OF THE BOX. THE BEAR BOX SLID DOWN THE ADJUSTING TRACK AND PINCHED INDEX FINGER ON RIGHT HAND CAUSING A LACERATION. EE WAS ADVISED NOT TO PLACE HANDS IN THIS POSITION.

EE was attempting to clear a frozen clog from hopper gate with a pry bar. Clog came free, forced the bar down pinning his hand between bar, belt and hopper gate.

Ee had just pulled cable up barge. cable was tight under cleat. Cable popped out from under cleat. Hitting him in the right upper mid leg.

Employee was installing a chute on the load out bin, utlizing a come a long. Come a long slipped & the chute twisted, which caused the ee's hand to be caught between the chute & the come along. Employees left index finger was severly cut and smashed. Injury resulted in stitches to the finger. Employee was wearing personal protective equipment, when the accident occured.

Employee was installing an air canon on the South Docksider North vessel with witness. A piece of pipe with flanges on both ends was threaded to the discharge valve of the air canon. When he pulled on the 40 lb pipe it came out of the threads and fell 4 ft onto his right foot. The pipe landed right where the end of the steel cap ends on his shoe and crushed his big toe.

EE WAS RUNNING BACKHOE TO REMOVE PIPE. HE STEPPED OFF MACHINE AND TWISTED RIGHT ANKLE ON ROCK.

Employee was shoveling under a hopper. It was a tight area and she used bad body position. She pulled a back muscle.

Fall from machine

Two haul trucks were parked bumper to bumper with 2 1/2' to 3' separation. Employee was climbing from one truck to another. He slipped and fell to the ground. EE HAD ARTHROSCOPIC PROCEDURE ON LEFT SHOULDER.

Cat 730 truck was stuck in mud on uneven ground. Operator had assisted in hooking up tow cables to haul truck. As operator was climbing back into the cab, the door slammed shut on his right thumb. Thumb had slight bruise. Operator was given medication for pain causing restricted duty for two days.

EMPLOYEE ALLEGED INSECT BITE. LOCATION AND TIME OF INCIDENT UNKNOWN.

Over-exertion in lifting objects

While lifting a 94 lb. bag of cement to stack on a cement pallet, employee felt a pop in his left shoulder.

VICTIM WAS PERFORMING REGULAR MAINTENANCE ON DRAGLINE, WHEN HE TRIPPED AND FELL ON THE CATWALK, BRUISING HIS KNEE.

Employee was lowering extension ladder into empty barge. Lower portion of ladder suddenly extended all the way down to bottom of barge. This causing employee's left hand to twist and causing injury mainly to his left thumb.

Employee was working in area 1. He was pulling on 6"" punch plate to attach chains on the crane. His left thumb was caught between the plate he was working on and the next plate up.

Employee was changing out belts on a conveyor drive motor, when the metal railing he had the come-along attached to broke, causing his finger to be pinched between the belt and pulley. The injury required stitiches.

Installing return roller (plant 7-grit belt)-employee struck roller with hammer to make it line up with bracket, little finger got pinched between roller and bracket, cutting finger.

Fall from scaffolds, walkways, platforms

An employee was doing welding on the windbox of the mill and had cut a small 8""x10"" opening in the catwalk platfrom to provide access below. A second employee was walking on the same catwalk planning on doing maintenance inside the mill. His foot fell into the hole, allowing him to fall, striking his arm on the open mill door.

While cleaning out underneath the primary crusher with a water hose, a small rock was thrown from the crusher striking the employee on the right side back of his head.

Employee was cleaning a plugged chute. A rock fell and struck his right ring finger.

ROBOTRAC HAD BAGGED DOWN FROM BEING OVERLOADED WITH COAL. EE WAS USING AN AIR LANCE TO FREE MATERIAL. BELT SURGED PULLING AIR LANCE AWAY FROM HIM. EE LOST FOOTING LOSING HARD HAT. AIR LANCE HIT SIDE OF MACHINE THEN CAME BACK AND HIT EE ON TOP OF HEAD.

WHILE CLOSING THE VALVE ON PORTABLE PROPANE TORCH, PALM OF HAND SLIPPED ON ROUGH EDGE OF NOZZLE, CAUSING LACERATION.

CLIMBING UP LADDER OF 100-TON HAUL TRUCK. (SLIPPED & FELL TO GROUND, HITTING RIGHT ARM ON ROCK. TWO LACERATIONS; STITCHES REQUIRED.)

Ee was holding a wrench on top of a bolt. When the nut was tightened. The Allen Wrench slipped and pinned his finger between the wrench and the side plate.

WHILE UNLOADING MILL SCALE AT RAIL UNLOADING, EE CLIMBED INTO CAR TO REMOVE MATERIAL THAT WAS BRIDGED ON THE WALLS OF THE RAIL CAR. WHILE ENTERING RAIL CAR EE STEPPED INTO HOLE IN MATERIAL CAUSING TO FALL FOWARD

VITIM WAS WEARING PROTECTIVE GEAR WHEN RESOCKETING A DRAG CABLE FOR A BE 1250W DRAGLINE. HE HELD ON TO THE CABLE TOO LONG POSITIONONG HIS MIDDLE FINGER AGAINST THE DRAG CHAIN. THIS RESULTED IN A FRACTURE AT THE FIRST KNUCKLE. EMPLOYEE HAD PREVIOUSLY SERVED NOTICE AND THIS WAS HIS LAST DAY OF EMPLOYMENT FOR THE COMPANY.

While putting belts on the coal mill fan, employee was rolling the belts on when his glove got hung between the belt and sheave at which time the tip of his left middle finger was pinched causing him to snatch his hand out tearing the tip of his finger off above the fingernail.

He was using torch to cut chain on floor of crusher when metal blew back in face.

Struck against stationary object

After going under a conveyor frame, the employee rose up into a structural I beam resulting in a broken nose, cut upper lip having 5 stitches, chipped front tooth and cut on forehead.

Was working on MPK hammer. corner of plate broke. attempting to reposition by hammering back into place. piece of metal was to small to be held in place by hand. struck thumb with 32 oz hammer.
