Struck by... (Not Elsewhere Classified)

Employee was struck on top of his head by a 4 x 8 sheet of plywood. He was wearing a hard hat at the time and did not seek medical treatment until Dec 23, 2014. He was released to return to work and was able to conduct his normal job duties. He continued to have neck pains and eventually elected to have surgery on June 15, 2015 making this a reportable accident.

Over-exertion in lifting objects

Employee was lifting a 5 gallon pail of oil from the back of a pickup truck to take to the oil containment area when he felt pain in his neck. He sought medical attention, diagnosed with a neck strain and is currently on restricted duty.

Caught in, under or between a moving and a stationary object

Labor Finders temporary employee: Placed hand toward a moving pallet and finger got caught and pinched. Resulted in seven stitches to the right ring finger.

Struck against stationary object

Employee was spraying out the bed of a haul truck when a hose clamp caused him to receive a hand laceration. He sought medical attention, received sutures and returned to full duty.
