Over-exertion (Not Elsewhere Classified)

Employee tripped backward over sample boxes. She did not fall but strained her back trying to keep from falling. She did not request medical care until 12/7/06.

Struck against a moving object

At end of shift on 11/17/06, employee reported that he had jarred his neck backing out of his first pass of pit cleaning and hit a rut. He refused examination that day. The next day, reported worsening pain, was taken to the ER and taken off work. CURRENTLY ONGoing.

Driver had spotted at the shovel and set the wheel brake. As she was placing her hands on the steering wheel, the dipper released the first load, jarring the truck, causing her hand to slip through the steering wheel. The steering wheel spoke struck her right hand.

Employee was operating a rubber tire dozer when he passed through a rough area and stated the seat bottomed out, causing him lower back pain. The employee was taken in for examination, given prescriptions for pain and released after one day off work.

Employee had been working on a haul truck, and when he dismounted the truck, he felt a pop in his hand. ONGOING He was seen by personal physician, referred to ortho clinic, and notified company he would be off work until approx. 11/3/06

While returning to operations at shift change, the van hit a pot hole and employee hit her head on the roof of the van, causing neck and lower back pain. He was wearing seatbelt. She was taken to the ER, referred to her personal physician, and has been taken off work until completion of physical therapy. ONGOING

Employee was driving out a pin with a hammer and it slipped and struck his left leg below the knee.

After dismounting the #386 dozer, employee took a step and went to the ground, rolling over once. Employee got up the second time and went down on the ground on his left side. Employee stated his right knee had given out and was hurting. He was seen by his physician and told to follow up with a specialist in 2 weeks. Ongoing.

Over-exertion in lifting objects

While lifting a heavily loaded garbage can, the EE's right shoulder popped resulting in extreme pain for the EE. He was diagnosed with a strained joint and taken off work until his next work rotation, missing his next shift, pm of 8/24. He reported to work on his next rotation, 8/27 and hurt too much to continue working and went home, called requesting to see a DR. ONGOING.

Fall from machine

Employee stepped off loader and missed the last step. He came down hard on his left foot fracturing 3rd and 4th toe.

Over-exertion in lifting objects

Employee was lifting a ripper point to install on a ripper & felt discomfort in his belly. He was taken to the ER & diagnosed with a hernia & taken off work. ON GOING>

EE was walking across the parking lot when she felt a sharp pain in her ankle. There was no slip trip or fall.She stated her ankle popped. She was transported to emergency room and diagnosed with a fracture. Currently ongoing.

Trk 112, retorque left rear inside wheel using RAD 30 torque tool. Holding RAD-30 with right hand @ trigger, left hand @ tube of tool. Caught left thumb between tube and wheel - reversed tool to pull hand out. Reported to tech, called Security, transported to hospital.

AS EE WAS COMING DOWN THE STAIR FROM THE CABLE REEL ON THE CABLE TRUCK EE STEPPED WRONG AND TWISTED LEFT ANKLE. GIVEN ICE PACKS, NEXT DAY SEEN BY HIS DR AND TAKEN OFF WORK UNTIL RETURN FROM DAYS OFF 4/6. 4/13 NOTIFIED SUPERVISOR EE RET'D DR 4/5 DIAGNOSED POSSIBLE FRACTURE, TAKEN OFF WORK-CURRENTLY ONGOING

Struck against a moving object

Water truck was traveling down E ramp when lost brakes operator tried to berm it on L side of the road to stop it. When Truck struck L berm ir swerved to the R and came in contact with berm on R side of road and turned over on driver side. EE had injured knee and some abrasions EE went to ER Dr gave him time off.EE was terminated

Mechanic was working on the engine on the 11 haul truck and had to come down the work stand to get a tool. As he stepped off the last step of the stand, he felt his left knee pop & get stiff. The mechanic cont'd to work but the paid did not go away, so he reported to his supervisor and was taken to the hospital to get checked out.

Struck against a moving object

A haul truck was traveling empty from the pit. It struck the berm on the pit edge with its left front tire. Skid marks indicated the truck slid about 4' as a result. The truck then traveled about 200' before stopping half way up the berm. The driver was taken to the hospital with visible head injuries. Initial investigation presumed he fell asleep and was not wearing his seat belt.

Fall from machine

While ee was servicing the TDD 388 dozer, he climbed on the right side of the dozer tracks. He leaned over to place tools on track. As he straightened up he lost his balance causing him to fall on the push arm then falling the rest of the way to the floor hitting his head. He was transported by ambulance. EE had a sprained knee & was given RX for pain & 1 day off work.

EE was washing a truck when the monitor spun as he turned on the water. He tried to hold it, jarring his back and sending a sharp pain down his back. He then complained of dull lower back pain. He was taken to the ER and removed from work. RTW 3/14/06.

Struck by falling object

Employee was loading railroad ties onto a flatbed truck with coworker. Coworker set his end down on the edge of the truck bed and came around to the other end to help push it onto the truck. The railroad tie slipped off the edge of the truck, causing employee to drop the other end. The railroad tie hit the employee on his right knee.

Employee was cleaning offices when she slipped on a brown wet spot on the floor. She tried to keep from falling and felt her chest pop. She did not seek medical treatment until 1/23/06.

Employee was walking on the leach dump and tripped on a valve causing him to fall. He landed on both hands and left shoulder popped.

Struck by flying object

While installing new launders beween the 3 & 4 flotation rows, a hydraulic ram and spacer were being used to expand sections into place. The spacer kicked out from the jack and struck the employee on the right eyebrow, close to the nose, required 3 sutures to close.

While using the one inch impact wrench to remove plow bolts from the cutting edge of a D-11, some type of foreign material entered or fell into employee's left eye.

Struck by falling object

Employee was underneath a flatbed truck wiring up the back-up alarm when dirt fell into his right eye. Employee was wearing safety glasses. ***DID NOT SEEK MEDICAL ATTENTION UNTIL 7/25/06**

Employee was cutting wood with a saw. The blade was dull and he wrapped his left hand around the base of the saw to push better and the saw pinched his little finger requiring sutures.

Employee stepped into a hole in the concrete outside the smelter scale house and tripped hitting his chin on the ground and cutting his lower lip with his teeth. He received 3 stitches and was given prescriptions and released to work.

Struck against stationary object

While removing cover off grease pump to mount pump on haul truck, employee's hand came in contact with sharp edge of stamped cover. Employee was taken to clinic & received four stitches, a tetnus shot and a prescription. Employee returned to work.

While preparing to don his safety glasses, employee hit himself in the eye with the stem of his glasses. The incident occurred on February 1, 2006 and employee was not taken to ER until the 2nd and was given prescription.

Employee was rolling a pipe over with a strap wrench when the wrench slipped striking his face.

Caught in, under or between a moving and a stationary object

The crew were laying down the rods, employee put his hand on the loop of the pulling plug and went to slide the tube down on the truck, at the time there was slack in the line when the tube was all the way down on the bed the slack pulled out of the cable causing the clevis to move back and pinch his finger between the clevis and the loop of the pulling plug.
