Struck against a moving object

Employee had spotted the 707 haul truck on the blindside of the 45 shovel, the shovel was filling its dipper behind the 707. As the dipper was coming up, the teeth made contact with the underside of the dovetail. Employee initially thought she was ok, but the following morning was in pain, contacted her supervisor, went to the ER, and was taken off work for 3 days.

Employee was performing cleanup in the shovel pit when he hit a bump and his seat bottomed out. his lower back began to hurt and he called his supervisor. He was taken to the ER and taken off work until 7/27/12.

When placing 2nd dipper into truck, rock rolled off dipper, striking the canopy. Driver left shovel pit, and later reported feeling pain in her upper right back and neck. She was taken to her doctor and taken off work. Currently ongoing.

Fall to the walkway or working surface

EE was cleaning station when she slipped and felling injuring her lower back and wrist. EE was taken in to Clinic, she was taken off work.

EE was washing equipment at the lube shop wash bay. As he was walking backwards he slipped on some mud resulting in him twisting his right ankle. EE initially refused medical treatment. On 4/13/2012 EE requested to be taken into clinic. EE was placed on restricted and released back to work.

Employee had climbed up a 3 rung ladder to a platform, when he put his left leg on first step to start going down it hyper extended.

Struck against a moving object

EE was directed by the dump operator to dump part of the load on top to repair a low berm. The operator pulled forward to allow the material to flow out, as the bed cleared the berm, the bed raised, then dropped to the level it had been hoisted, jarring the ee. EE was given restrictions until cleared by PCP. Restrictions could not be accommodated. Ongoing.

Caught in, under or between a moving and a stationary object

Employee was replacing a belt on a motor on a locked out agitator. After replacement employee turned pulley causing his right thumb to be pinched between the belt and pulley. This caused a fracture of the bone at the tip of his right thumb.

When starting to load 728 haul truck, the dipper door came open above the dovetail, the material caused the bed to raise up and bounce the operator. The operator complained that his neck hurt. He was taken to the ER, and taken off work until seen by PCP. He lost 3 days of work, then released with light duty restrictions, currently ongoing.

EE was helping reposition a ball bucket when her foot was pinched between the ball bucket and the stairway. EE was taken off work.

Over-exertion in lifting objects

On 4/11/12 EE was picking up a cable when he felt a pain in his right shoulder. EE reported incident to fill on 4/12/11, but did not want medical attention at the time. On 4/18/2012 EE requested to be taken in, stating his pain had worsened. EE was placed on restrictions.

EE reached to grab handrails to board the platform on his haul truck, as he was stepping up his foot slipped off the rung causing him to extend his shoulder. EE was taken to the clinic and released back work with restrictions.

Installing hub seal with another employee. Repositioned hand when dead blow hammer stuck left ring finger resulting in fracture and stitches.

Employee went to help another employee trip the trash dumpster and while he was tripping the dumpster, he stepped wrong and twisted his ankle. He was taken to the ER and place on restricted duty. Currently ONGOING.

Struck by falling object

While servicing the RD13, employee was asked by driller to assist in lifting a bushing. once lifted, the bushing fell back in the hole on the deck and smashed the employees right index finger between the bushing and the deck. he was taken to the ER and given five stitches to close the wound. Released to light duty beginning 8/3/12.

Employee was walking upstairs to the south thickener to do her normal rounds when she felt her knee pop out of place. She was taken to the ER, a brace was put on her knee, and she was referred to an Orthopedic doctor, now waiting on MRI, released to work with restrictions. ONGOING.

Over-exertion in lifting objects

While taking mill samples employee was lifting his 22 lb. sample bucket to take to the Met Lab and twisted his back the wrong way, causing pain in his lower back. He was taken to the ER, given and restricted duty.

Employee was bending over to fill out paperwork as he stood up right he strained his lower back.

EE was operating a haul truck and was being loaded when she was jarred. EE notified her supervisor that she felt a tightness in her back. EE initially refused medical treatment. On 4/13/2012 EE came to work and requested to be taken into clinic. She was placed on restrictions and released back to work.

EE was standing up from a sitting position when he felt his knee pop. EE initially refused any medical treatment. On April 25th 2012 EE called his supervisor on day off to inform he was going to be seen at the clinic for his knee. EE was placed on restrictions.

EE was getting out of his vehicle when he twisted his left ankle. EE was taken to the clinic. EE was released back to work with restrictions.

EE was operating dozer on dump when he was jarred. EE notified supervisor EE was taken into clinic and placed on light duty for 2 days.

Installing turbo gasket on 793B haul truck engine when turbo moved and pinched finger between turbo and exhaust gasket.

Employee had finished inspecting equipment and was walking when his ankle buckled causing it to twist. Employee tried to walk off the injury as he continued to work, toward the end of his shift he noticed more discomfort. Employee was looked at and placed on restrictive duty.

Struck by flying object

Employee turned on the A/C in his equipment and dust blew out the vent, some getting in his eye. He attempted to flush his eye, but his eye continued to hurt. He was taken to the hospital, where he was diagnosed with a corneal abrasion. Antibiotic ointment and a patch were applied. He was release to his next shift.
