Accident type, without injuries

Shift supervisor reported power pole at the K11 substation was on fire from electrical arcing, presumed to be due to the accumulation of snow. 12:51 shift electrician cut power to substation. Once the arching subsided flames went out but pole continued to smolder past 30 minute time frame. Smoldering extinguished at 2:05. At approx. 1:10 MSHA was notified.

Accident type, without injuries

Night security guard noticed lightning, then at 4:11 noticed the flames. Lightning had struck a power pole next to the reclamation offices near tailings area. The pole was burned beyond use. No damage to the building and no personnel were working in the offices at the time. The fire was extinguished at 4:45.

Struck against a moving object

Employee was on the 703 haul truck, hit a bump in the road which caused the truck to bounce. Employee complained the seat belt buckle caused pain in his hip. He was taken to the hospital and removed from work until released by personal physician on Dec. 9, 2011. RTW next scheduled shift on Dec. 11, 2011.

Fall to the walkway or working surface

While walking to personal vehicle in the employee parking lot, the employee's states his ankle gave way causing him to fall and land on his elbow.

While operating a dozer the employee back over a rock and jarred his back.

Employee was driving haul truck in 5th gear and hit a bump, and felt pain in his lower back.

Over-exertion (Not Elsewhere Classified)

Operator of the RTD exited the cab at the Southside Queue. As he approached the top of the stairs on the rubber tire, he felt his left knee pop and felt pain in the knee. He was taken to his physician, given a brace and crutches and an MRI was scheduled. Taken off work and currently ONGOING.

Struck against a moving object

Employee was operating a dozer and while backing up he hit a rock and jarred his neck.

Struck against stationary object

While stepping off the service truck, employee stepped on a rock and twisted his left ankle. He was taken to his physician and taken off work. Currently ONGOING.

The truck operator was being loaded and the loader bucket struck the bed of the truck. This jolted the employee and caused a muscle strain to his upper back.

Employee reported hitting a depression in a haul road on the stockpile, and the seat bottomed out as well. He did not notice immediate pain, but about an hour later, contacted his supervisor wanting to be examined. The ER physician took him off work for his last shift. On his next scheduled shift, he called in saying he needed more medical attention. ON GOING

Fall from machine

The employee was exiting a transportation van at the tailings dam pond 4 location. It was still dark and ground area was level. Upon exit, the employee fell to the ground. The employee had pain in her right lower leg.

The employee was starting to walk down an incline into the Sag #1 feed tunnel; he slipped on some mud and fell on his back and right shoulder. He was in pain and taken to the emergency room where he was diagnosed with a dislocated shoulder, and taken off work. ONGOING

Fall from machine

At the end of shift the operator had opened the door of the loader unsure if he had latched the door back and stepped onto the first step to get off the 966 loader. A large wind gust came up and slammed the door shut onto employee knocking him off the loader onto the ground onto his back.

Caught in, under or between a moving and a stationary object

An SXEW employee was unloading a pump off of the warehouse pickup with a forklift as a warehouse employee spotted from the side of the pickup. While pulling the pallet, the pump got hung up and the second employee tried to prevent the pump from tipping, got his arm caught between the pump and truck bed, fracturing his wrist. ONGOING

Employee slipped on ice and fell, hitting his right elbow, and the employee felt a pop in his shoulder which caused pain. He was taken to his physician, then an orthopedic doctor and was placed on restricted duty with a sling until a follow up appointment. ONGOING.

Caught in, under or between a moving and a stationary object

Employee was getting into the 1074 van at start of shift. While attempting to enter the van through the side door, employee put her right hand in the door frame of the front passenger door. Another employee closed the door, catching the first employee's hand in the door, causing bruising. Employee was given restricted duty. CURRENTLY ONGOING.

Caught in, under or between a moving and a stationary object

Installing CAT 793 rear strut assembly. While trying to install upper pin into top cylinder bore, holding steel wedge to support the strut the strut shifted and pinched hand resulting in crush /laceration of left index finger.

Absorption of radiations, caustics, toxic and noxious substances

Employee found a leak on inlet line to cell, locked out line and began to repair it. As he cut the 2"" PVC line, the lined rocked back and forth, causing the glued part attached to the valve to come off, spraying employee in the face with solution. He was taken to the doctor, given a prescription and returned to work.

Struck by flying object

Employee states he was leaning over to take a sample out of the bag when he was working at the cyclone and felt something in his right eye so he rubbed eye.

The employee was using a sledge hammer to remove a track link when a piece of metal dislodged and embedded in his left forearm. He was taken to the hospital and the metal was removed the laceration required one stitch.

Struck against a moving object

A 998H loader was being used to load 740 haul truck. When rolling the bucket out of the bed, a bucket tooth hung on the bed causing employee to move around in the cab.

Struck by falling object

Employee was working under walkway and looked up, dirt from the walkway above fell and got into employee's right eye. He repeatedly flushed his eye, was examined by an EMT then taken to the clinic and was given a prescription, and returned to work.

Employee was working under and inside CAT 793 haul truck frame attempting to loosen hose connection with wrench. Hose fitting broke loose resulting in wrench hitting employee on lip. Laceration to lip and broken tooth resulted in stitches and dental work. No lost time or restrictions resulted.
