Employees were changing out crowd brakes on P&H Shovel. Employees states when last bolt removed, shovel shifted and knocked them into ladder. Employee left work without reporting incident. 02/13/26 employee notified Supervisor they were at ER, with neck injury and taken off work. Currently ONGOING.

Employee operating a dozer when it came down dig face backwards, resting on cab. Employee released seatbelt and fell against cab, causing discomfort in shoulder. Taken to ER, released to next shift; before RTW, saw PCP, taken off work until seen by surgeon from prior back/neck surgery to be cleared. ONGOING.
The employee tripped and fell off last night around 9:30 PM, EE called to report when EE was back in the El Paso terminal, then EE called the nurse this morning to notify of having some difficulty to breathe. I asked them to please notify the mine due to the injury.
Employee was called to assist in cleanup of a floc spill; Employee was using a shovel to clear the floc, slipped and fell, hit the right side of their face on the concrete, causing a laceration in right eyebrow, required sutures.
Employee was stripping/cutting the exterior jacket of the 25kv cable when the insulation removal tool (knife) slipped and cut across the top the left index finger.
