Bodily reaction, (Not Elsewhere Classified)

Employee was bending over closing the drain on the vacuum system in the X-Ray sampling room and when he tried to stand back up, the employee said that he felt pain in his lower back and could not stand up straight.

Struck against a moving object

While operating #4 637 scraper, on the south side of pond C loading and dumping at the catch pond below, operator hit bump and injured right shoulder.

Fall onto or against objects

Employee was mounting the haul truck and slipped on the third step down from the top and hit her knee on the steps. She had a small scratch and bruise, and was complaining of pain in her upper leg and lower middle back. She was taken in for examination at the ER, diagnosed with back strain, referred to her personal physician and taken off work. ONGOING.

Over-exertion in lifting objects

Employee was installing a dipper stop; as he lifted it to install it he felt his wrist pop. He was taken to his doctor, diagnosed with a sprain, given a soft splint and referred to an orthopedic specialist. He was also given a prescription for pain and a 5 lb restriction for his right hand.

Over-exertion in pulling or pushing objects

Employee was servicing equipment, he was pulling fuel hose from service truck to loader, when he felt a pain in his lower right back and hip.

Employee was coming off 140 H grader maintaining 3 point contact. When he got on the ground he felt burning pain to lower abdomen area.

Struck by falling object

While placing a steel plate in place as a barrier, the bolt was placed in wrong and the plate slipped and hit the employee on the left foot, fracturing his metatarsal.

While helping mechanic on 777 dash board, employee reached to secure a brass nut on a 1"" by 2"" control valve. Employee had a sharp pain to lower back.

Employee was operating a D9 dozer and throttling forward pushing a load. His left hand was on the throttle control and he drove over a small drop off on the ground, jamming his left wrist against the throttle control. He was taken to the clinic, and referred to a specialist for evaluation to determine if he has a tear or hairline fracture in his wrist. Ongoing.

Employee was directing a skid steer operator in lifting a metal saw horse when the saw horse slid off the bucket. The sawhorse landed above the safety toe of his left foot causing a deep contusion.

Employee was working on the #34 shovel removing plastic sheeting used to cover equipment during an overnight storm. The sheeting had blown down during the night, and covered an open access hole on revolving deck. The employee stepped onto the plastic, and his left leg fell through the hole up to his thigh, causing a deep contusion to the outside left thigh.

Employee was pulling (hanger) bars when he first felt the pain in his lower back. When he got on the forklift is when he was in a lot of pain. He was taken to the doctor, and taken off work for 72 hours, until seen by PCP. PCP extended the time off. Currently ONGOING.

Employee waw taking antifeeze over to the dozer area. While standing near the dozer station area, he took a step to the right and rolled his right ankle. He was taken in to be checked and taken off work for approx. 4 wks. Currently ONGOING.

On 6/10/08, employee notified his supervisor that he might need the next day off as his back was hurting from helping the UPS driver unload the daily delivery on June 5th. EE was sent to see his physician who has requested futher testing. Currently ONGOING.

Employee was checking vertimill cyclone apexes in underflow tub and got stuck in mud, strained right leg and knee while attempting to free himself.

When the shovel released the swing brakes, the dipper opened dropping material on the dovetail. Initially, the driver thought she was ok, but about 2:15 a.m. called her supervisor, complaining of back pain. She was taken to the ER, diagnosed with lower back strain, and taken off work until seen by her personal physician. ONGOING

Truck was being loaded by loader.The 2nd bucket had a rock in it,which jarred the EE and broke the windshield.The EE told his supervisor he felt fine and took the truck to the shop for windshield replacement.When he bent over to pick up the chock,he stated his lower back began to hurt.He was taken to the clinic and taken off work several days. ONGOING.

EE was being loaded at the shovel.Last dipper opened and landed on dovetail,jarring EE. She was taken to the clinic for back and hand pain, diagnosed with cerival strain and released to her next shift 5/11. On 5/12 she called in saying she was going back to the doctor on the 14th. She was taken off work until 5/22.

Over-exertion in pulling or pushing objects

Employee was pulling cable up on the berm in the shovel pit when he felt a strain in his back. He reported the incident to his supervisor but assured the supervisor he was alright. Later in the shift, he complained of more pain, was taken to the ER, diagnosed with lower back strain and taken off work. Currently Ongoing.

Contact with electrical current

The operator of the 121 haul truck pulled into the K86 to delay for a blast. The operator exited the cable to observe the blast, and leaned on the grid box and received an electrical shock. He was taken to the ER and taken off work for 2 days. Currently Ongoing.

Fall to the walkway or working surface

Employee was in front of the #3 Ball Mill basement floor float trunnion. As he went to leave he slipped on oil on the floor, fell and hurt his left shoulder. He was taken to the ER and taken off work until he can be evaluated by his personal physician on 2/25/08. ONGOING.

Driver of haul truck was backing to the shovel, shovel operator tripped the dipper. At the same time the driver was trying to take the truck out of propel. The movement caused the driver to jam her thumb into the shift selector. She noticed swelling and pain, was taken to ER for XRays and taken off work for 2 days.

Fall from machine

As employee was finishing with refueling of excavator the employee slipped on muddy track and fell to the ground approximately 3 to 4 feet in height. Employee fell on right side of body and experienced sore shoulder.

The flotation crew was in the process of installing liners on the spare vertimill screw with the screw suspended 4-6 inches above the ground. The brakes on the crane failed, causing the screw to sway pushing the employee against a crossmember in the stand. He sustained a scrape and contusions on his right shoulder and chest area.

Haul truck driver dumped his load and had to pull forward a little so the material would clear the bed. As he did, the bed dropped some shaking the truck and jarring the driver. He was taken to the clinic, given restricted duty. He attempted to work the next day, complained of pain, returned to the clinic and given more restriction. Currently ONGOING.

Employee was on his knees checking the PH levels in the pond. Afterward his knee began itching and later that night he discovered a bump with pus. He performed first aid throughout the weekend, however, the infection worsened and he requested medical care on 3/10/08.

Cutting Plastic Liner with Utility Knife, Knife slipped and cut right knee

Employee was washing on the feeder floor at the primary crusher. He turned around to wash a different spot and felt his ankle give and rolled it. He was taken to the doctor, diagnosed with strain, and given restricted duty until 08/15/08. (Same ankle employee injured in 2007)

Employee was torch cutting a 2' x 3' section of tank wall when as he neared completion of cut the cut out piece began to swing toward him. He moved himself to get out of the way but as it swung it caught his lanyard and pulled him back into it. The piece struck him on his left knee and stretched the tissue that supports the knee.

Over-exertion in lifting objects

Employee was lifting two cases of Gatorade at one time and made a fast jerking motion to lift and injured his back. He was taken in to be examined and given 2 days restricted duty.

Caught in, under or between a moving and a stationary object

Worker was removing the inside lock ring. While lowering it to the ground, it slipped and caused his left index finger to rest between the lock ring ends. When the lock ring hit the ground, the two open ends closed smashing his finger. The doctor removed him from one day, returned the 2nd with restrictions & therapy currently going on.

The employee had just washed the drill shack floor. After exiting the drill shack, the helper slipped and hit his arm on a drill rod.

Employee had just exited the man van and was walking towards her haul truck when she said the ground sloughed beneath her foot, causing her to roll her ankle. She was taken to the ER (due to time of day) put on crutches, given a soft cast and restrictions until 5/7/08.

Employee was exiting a toilet stall prior to heading out to the pit to start work, when er knuckle hit the latch and the bottom part of her finger got caught in the latch. She didn't notice pain at the time. Approximately 11:45 she contacted the supervisor complaining of pain. She was taken to the ER and given Restricted duty.

Employee was fusing pipe and tripped on a rock. When he fell his hand got caught between a pipe and the ground spraining his left wrist.

Enployee was climbing down the ladder on right hand crowd platform when he felt his ankle twist. He was taken to his physician and given light duty. ONGOING

Operator was placing a single 32 inch by 40 foot pipe on top of another. Spotter placed a brace on the left side and went over to place a brace on the right side. He signalled to lift load a little more. Operator saw the brace through the front the operator thought it was good to bring the load down. He still had hand under the load and 2 fingers on left had were mashed.

Employee was mopping the ladies restroom when the end of the fiberglass mop handle poked the employee in the right eye. Employee was working at a faster pace due to not completing assigned areas from the day before.

EE was preparing some duct work for install when he proceeded to align duct with his right hand slipped cutting his wrsit with a sharp corner of the duct. all ppe were being used.

Employee was stepping off the FM 14 Hydro Crane, and his hand slipped on the door. The door compartment did not have the rubber liner on it, and he cut his hand between the ring and pinky fingers on the door tin. The laceration required stitches.

While employee was setting up to unload lead anodes, he felt somthing in his left eye. The eye was flushed immediately, and he was taken to the clinic where the optometrist removed a foreign object from his eye and he was released to work.

Employee called her supervisor and reported that she has something in her eye. The eye was flushed several times, and still bothered her. She was taken to the doctor and diagnosed with corneal abrasion. No mechanism of injury could be established, employee was wearing safety glasses with sideshields and the windows were closed.

Employee was struck on the rt side of his face by a wrench on 4/9 while trying to remove a coupling using portapowers, but did not go to the doctor due to lack of visible injury or pain. On 4/18, his wife noticed his eye was not dilating. He was taken to the clinic, diagnosed with inflammation and swelling of the eye ball, given drops, and returned to work.

Struck by flying object

Employee reported on 1/24/08 that he had got something in his eye on 1/23/08. When he woke up on 1/24/08 with redness and irritation and noticed something on his cornea. He was taken to the ophthalmologist who removed the foreign body and prescribed an antibiotic. The employee returned to work with no restrictions.

Inhalation of radiations, caustics, toxic and noxious substances

Employee drives a pickup around property checking all offsites for concerns. Is in and out of the vehicle. Employee was experiencing chest pains and headache. He was taken to the local hospital and diagnosed with elevated carbon monoxide levels. When the pick up cab was monitored, it confirmed the diagnosis.

At approx. 10:00, employee was driving haul truck to shop for lube service, and was not feeling well. He went to gen. supervisor and requested a 1st responder check his vitals. He then requested to be transported to ER at local hospital. On 4/7, supervisor was notified that he was going to be removed from life support. At that time, company notified MSHA.
