Accident type, without injuries

Matting timbers were used to support shovel side frame caught fire which lasted more than 30 min. Presumed to have been caused by slag from welding/cutting torch the previous afternoon. Work was completed at 3:00 pm 11/23, final walk around at 5:45 pm. At 4:30 am 11/24, fire noticed by night shift personnel. MSHA notified at 5:02.

Fall to the walkway or working surface

Driver was walking to her truck. Her foot got stuck in the mud and she fell forward. She twisted to keep from landing in the mud face first and landed on her left side. When she got up, she felt pain in her left hip. She was taken to the hospital and diagnosed with a fractured pelvis.

Over-exertion in pulling or pushing objects

A maintenance technician was in the process of loosening a hose on a pump. As the mechanic pulled on the wrench, the fitting came loose unexpectedly and the mechanic felt a pop and pain in his right shoulder. He was taken to his physician, given restricted duty til 10/08/10. On 10/08/10, he returned to the physician, restricted duty was extended pending an MRI. ONGOING.

Rubbed or abraded

Required PPE is a rubber boot for the area worked in. He was given a size 10 boot when he needed a size 10.5. He wore the boot instead of reporting the incorrect size. The boot rubbed a sore on the end of his left big toe that later became infected. He was given a daily antibiotic and a shot of antibiotics and released full duty.

Struck by... (Not Elsewhere Classified)

Employee was exiting his vehicle in the employee parking lot when he arrived for work and poked himself in the eye with his newspaper. This caused a painful scratch on his eyeball. He was taken to the emergency room, antibiotic ointment and a patch were administered, and he was told to take the rest of the day off. He was released to work on his next regular shift.

Contact with hot objects or substances

Tig Welding rod slid off left hand coming into contact with the right forearm, burning through several layers of clothing and burned employee's forearm.

Personal Illness. Employee was surveying when he became dizzy and he sat down on the berm. The employee was observed by a co-worker who went to the employee to check on him and he immediately called the supervisor to come to the area where the employee was located. A First Responder was contacted and a Mayday was called. The employee was transported to the area hospital.
