Struck by flying object

The employee was present while two contract workers were performing a repair on a centrifugal water pump. During the repair, an explosion occurred. Heat may have been applied during the repair. The employee suffered fatal injuries.

Repair of RS Conv 99% complete w/severe storm minutes away. 2 riggers supplied by Edwards, Inc on catwalk loading tools on deck of hyd. manlift supplied by Hanson Agg. 1 rigger thrust a heavy steel pinch bar on deck of manlift & it slid across deck, under entrance w/no kick plate, fell onto conv. carriage, glanced toward EE who threw rt arm up as shield, hit his arm & broke it.

Employee was tightening bolts on a jaw crusher with a knock wrench. The wrench slipped from the bolt and struck him in the arm. Initial treatment was first aid. Follow-up visit on 06/02/2014 resulted in the application of a removable wrist splint. Employee was also transferred to another operator's tower not requiring the use of a joystick controller.

While lowering a snubber pulley into a position where it could be broken down for repair the chain holding the drive side of the pulley slipped causing it to drop about 2"" pinching employee's hand between the bearing housing and conveyor frame which resulted in his finger being fractured.

Over-exertion in lifting objects

During the process of lifting and pulling a 3"" poly discharge hose from a position in the man lift to remove water from the south side of the creek in a sump area the employee through he experienced a potential back related problem. He thought that it might get better overnight but was still experiencing a strain the following morning.

A pry bar slipped while changing out a roller on a wash screen SP38 feed conveyor and hit the right wrist.

Struck against stationary object

EE was dismounting from his 773 haul truck and stepped backwards away from his ladder and stepped on a 4"" x 4"" block. EE then lost his balance and contacted the maintenance shop wall with his hand and cut his hand. EE immediately called management over the Motorola radio.

Struck against a moving object

A mining foreman was operating a Cat D6K2 LGP dozer blading a haul road. While blading he struck a rock, buried in the subgrade. This stopped the machine suddenly, causing the operator to lunge forward and strike his head against a steel grab bar in the machine. Injury resulting was a left forehead laceration above the left eye, requiring 10 stitches.

Struck by falling object

Employee was under the jaw crusher, removing the tension rod pin by striking with a hammer. With the first blow, collected dust and debris was dislodged from above and fell into his face.

Employee was changing skirt board rubber on conv. 30. Has he was sliding the rubber back in, his hand slipped and hit the knife that he placed on the shelf above where he was working.

Annual audiogram revealed a Standard Threshold Shift on an employee. Subsequent re-tests confirmed the shift.

While reporting to work, an employee of a tenant suffered a fatal cardiac episode. The tenant is not associated with Hanson Aggregates and is not involved in the mining process.

At an area on the property but not at the location of the mining operations, an abandoned, burned car was discovered by the Plant Foreman and a Contractor. Upon further investigation of the vehicle, remains of a deceased victim were discovered in the trunk. Police investigators concluded the victim and automobile had been disposed of at the site.
