KNOCKING METAL PIN OUT, WHEN PIN CAME OUT METAL DROPPED PLANT 1"" AND PIPE USED FOR KNOCKING PIN OUT JUMPED UP AND STRUCK HIM IN CHIN. IT REQUIRED 4 STITCHES.

Fall from machine

EMPLOYEE WAS CLIMBING DOWN LADDER FROM THE WATER TRUCK CAB WHEN HIS FOOT SLIPPED FROM THE BOTTOM RUNG CAUSING HIM TO FALL TO THE GROUND. THE GLASSES HE WAS WEARING CAUSED A SMALL CUT JUST ABOV E HIS LEFT EYE REQUIRING 2 STITCHES TO CLOSE THE WOUND.
