Struck against a moving object

driver was backing pickup along the outside edge of embankment got to close and slide down embankment, the seat belt caused to strain muscle in left shoulder.

Struck against a moving object

The employee was operating an articulated truck. Employee stated employee fell asleep. The tire tracks indicate the truck rode up the left side embankment causing the cab to turn over on the right side. Employee received a concussion without loss of consciousness and a laceration to the right ear without foreign body.

While removing material from the loading face, the pit loader came to a sudden stop, ""jolting"" the machine. Later in the shift, employee reported some discomfort between the shoulder blades. First aid/self care measures were employed. A clinic visit was required on 9/23. At that time, the employee's work duties were restricted from operating equipment due to medical precautions.

Caught in, under or between a moving and a stationary object

Miner was installing channel to structure. Channel was supported on fork lift forks and secured w/a chain fall. Channel slipped off forks pinching the miners right hand.

Struck against stationary object

Employee initially injured L shoulder when shoulder struck a beam when pry bar slipped. Checked out and released without restrictions. Early September reported continued discomfort. Medical evaluations reveal injury to rotator cuff. Specialist visit on 9/17 suggests surgery on the shoulder on 10/2/2020.
