Employee was retrieving scrap parts from the bed of a service truck. When the employee bent to pick up a part weighing approximately 10 pounds, suffered a lower back strain.

Employee was in the process of lighting a cutting torch during routine maintenance activities. Seconds after lighting the torch, there was a loud noise nearby on the coiled hose reel. A rupture in the oxygen hose produced the noise, and due to proximity, resulted in the employee having ringing in the ear and subsequent temporary disorientation.
In process of removing cheek plate from jaw that was binded (jammed) up for removal, attached crane to cheek plate and applied pressure and cheek plate popped out and swung 5 feet and caught welder fabricator in face and knocked EE backwards. Worker got bloody nose and 5 stitches. Went to doctors and told to take day off.
Injured employee was removing a wear plate from a feeder. As a weld was being cut free with a torch, a secondary weld failed, allowing the steel plate to release, striking the employee on top of the left foot. The employee sustained broken bones to the left foot.
Employee was welding screen 1 roolback plate when EE noticed some irritation in right eye. EE removed contact and kept working. EE went home and eye began to swell. EE texted supervisor and reported the issue. EE was taken to ER which referred EE to eye doctor.
On 3-7-23 While hammering screens into place of scalping screen, employee missed the screen and contacted EE's finger with the hammer. EE was taken to the clinic and received 4 stitches
