Inhalation of radiations, caustics, toxic and noxious substances

EMPLOYEE CUT A POLYDEX SCREEN PANEL WITH A CHOPSAW. HE BECAME ILL LATER THAT EVENING AND DEVELOPED A MILD COUGH. 2 DAYS LATER HE WENT TO THE EMERGENCY ROOM BECAUSE OF SHORTNESS OF BREATH. HE WAS DIAGNOSED WITH CHEMICAL PNEUMONIA, CAUSED EVIDENTLY DUE TO FUMES FROM THE SAWING OPERATION.

Employee stepped out of truck onto truck's fuel tank. Employee's right heel hung in the step, causing employee to twist foot, which resulted in a right ankle sprain.

Two employees were loading a new s rod on #2. Chain holding rod slipped and rod dropped and smashed one employee's left hand.

EMPLOYEE STATED THAT HE ATTEMPTED TO ROLL A ROCK FROM THE PATH OF HIS TRUCK. AS HE STOOD, HE FELT A ""POP"" SENSATION IN HIS RIGHT BICEP. HE WAS TREATED FOR A TORN BICEP MUSCLE.

A LOCKPOST WEDGE IN A 4 1/4 CRUSHER WAS EJECTED FROM THE LOCKPOST DURING AN ADJUSTMENT PROCEDURE, STRIKING THE EE ON THE RIGHT SIDE OF HIS FACE. HIS SAFETY GLASSES ABSORBED THE IMPACT LIMITING THE INJURY TO THE EYE ORBIT AREA. THE RESULT WAS CONTUSION & MINOR LACERATION, BUT NO DAMAGE TO THE EYE. RESTRICTED DUTY.

The employee was using a sledge hammer to dislodge jammed rock located in the jaw feeder's chute. He was hitting the outside of the chute with the sledge hammer when his knuckle struck (caught between) the chute causing a small gash on his right index finger that required stitches to close the wound.

INSTALLING MOTOR ON JAW FEEDER, EE REPORTED LOWER BACK PAIN. PHYSICIAN PLACED HIM ON RESTRICTED DUTY THRU MARCH FIRST.

WHILE PUSHING ON A RIP-RAP GATE, EMPLOYEE'S FINGER WAS CAUGHT BETWEEN THE GATE AND A PRY BAR USED BY A SECOND EMPLOYEE. EMPLOYEE'S FINGER WAS MASHED.

After cutting a piece of steel, the employee picked it up to load it onto a truck. As he picked it up, he rubbed his arm against the edge of the steel plate, causing a 1"" long laceration on his forearm. He was given a tetnus shot and treated with derma-bond. He returned to work the same day.

Employee was assisting in the moving of a crusher eccentric, when he mashed his finger between the eccentric and a frame, resulting in a laceration to his finger.

Supervisor was holding a box wrench on the bolt heads of the tube assembly of a Telsmith Screen while another employee was tightening the nuts on the opposite side with a ratchet when the Supervisor's right little finger got pinched causing a laceration that required five stitches.

WHILE REPAIRING A WATER LINE FOR THE 52FC CRUSHER THE WIND BLEW A PARTICLE OF GRIT INTO EE'S LEFT EYE. WASH EYE OUT WITH WATER,EXPERIENCED IRRITATION. EXAMINED BY DR & PRESCRIBED ANTIBIOTIC EYE OINTMENT. WAS WEARING SAFETY GLASSES AT THE TIME.

THE CONTRACT MECHANIC WAS RIGGING A HYDRAULIC HAMMER TO BE LIFTED. WHEN HE TOSSED THE CHAIN OVER THE HAMMER IT SWUNG BACK & CONTACTED HIS FRONT TOOTH W/THE LIFTING LUG. THIS CONTACT RESULTED IN A SERIOUS CRACK TO THE MECHANICS FRONT TOOTH.

On 1/26/05, employee complained to plant manager he had pain in his left arm, later diagnosed as tennis elbow. Employee stated he used a 3# mall on the morning of December 8, 2004 to remove a belt splice, and had noticed aching feeling afterwards. Employee did not report incident or pain until 1/26/05.

Hanson was notified that employee was diagnosed with Carpal Tunnel syndrome, which the physician determined it was probably work related.

While running rock bolter ee reported numbness on left side of body. Ambulance was called and ee was transported to hospital. Was diagnosed as having a mild heart attack. Ee returned to work with no restrictions two days later. EE scheduled to return to physician for additional tests as he suffers from high blood pressure.

A CUSTOMER truck driver employed by Autry Grading was removing a rock hung in the tailgate of the truck before being loaded. The tailgate slammed on her left index finger, causing a crushing avulsion of the tip of the finger.
