Fall from machine

Employee was exiting a Cat D-8 dozer when EE missed the bottom step and fell approximately 2 ft. to the ground. EE sustained a sprain to the rt. wrist. Possible unspecified fracture. Diagnosed on 8/24 as a fracture to the wrist requiring a cast.

Employee was passing a power drill to another employee when EE accidentally pressed the trigger. This caused the drill to rotate, striking the employee's left ring finger, resulting in a fracture to the finger.

The employee was cutting a cardboard tube used to keep debris out of the blast hole, but the tube was difficult to cut and slipping. Employee was using a safety knife with debris in it and it slipped off the tube, didn't retract, and cut the employee's right forearm. Employee needed 8-10 stitches.

EE was repairing a tear on T1 conveyor belt. As EE was removing the bad belt EE's locking blade knife slipped and cut left forearm. Injury resulted in 5 stitches. EE was wearing cut resistant gloves
