Contact with electrical current

EE was found unconscious, with no resp or pulse pit between hydroseparator and floc building. CPR was administered and EE was transported to CMS where he was pronounced dead. Autopsy report has not been recieved. A heat trace tape was found arcing near where EE was found.

Struck against stationary object

EE was driving under a belt cross while pulling a fan with a toolcat. There was loose muck on the floor and he lost traction. He backed up to get better traction. When he was about halfway through, the front tires caught loose muck, which drove Employees head between a return roller and the belt.

EE had just finished taking off covers of the miner to do p.m. work, as he finished up his rountine work he stepped backwards and tripped on the covers causing him to fall on his side.

Over-exertion (Not Elsewhere Classified)

EE was sitting down trying to pull a nip off a transformer but couldn't do so and as he rolled over to get his knees to get up he felt a sharp pain in the middle of his back.

Fall from ladders

EE was climbing a ladder to tie the ladder off; the ladder shifted and started to fall. A spotter was holding the ladder but could not hold it when the ladder shifted. The ladder and employee fell approximately 12 feet to the ground, with the employee landing on top of the ladder.

Over-exertion in lifting objects

EE was shoveling spillage around the 6th level #3 tail and as he twisted his back to throw muck on the belt line he felt a sharp pain on his lower back.

EE was picking up old conduit on the ground floor of the screening plant. She was taking old pieces of conduit to a dumpster, her feet became entangled in wiring that was hanging out of the end of the conduit. She fell to the ground, injuring her back.

EE was driving ram car, as he got to the w301 air doors he started to get up off his seat in order to get off the car and as he did so, he jammed his neck on a fan switch which was directly over him.

While walking to the rest room, Jose slipped on a uniform clothes hanger that was on the floor around the dirty uniform barrel containers. No medical attention was initially required. Subsequently-EE lost 29 workdays.

Over-exertion in lifting objects

Employee was doing some housekeeping and as he went to lift a car battery off of his vehicle he pulled a muscle in his back.

Inhalation of radiations, caustics, toxic and noxious substances

Enclosed area-no damage or impairment.

Employee was lifting the lid to the cable reel compartment; while doing so he felt a pain in his right lower abdomen and fell to the floor.

Caught in, under or between a moving and a stationary object

EE working on drive and went to place 4ftX8ft. guard back up. He placed one end of the guard back on the pole bracket and then went to lift the opposite end. He lifted the guard up, the other end that was set slipped off the bracket and caused him to bring his guard up to the back and then straight down. Pinched his thumb and index finger against the pole bracket, fracture.

This incident with EE was just a precautionary report until June 27th 2008, when he went in for surgery on his left shoulder, that is when it became a lost time. At that time, a 7000-1 should have been submitted and it was overlooked.

EE stepped back towards jeep from inside drive area, his foot slipped on a piece of belting on the roadway, causing pain to his right knee.

EE was in process of lubricating the tail roller bearings, he walked behind the tail roller and slipped on spillage off the tail roller, landing on his upper body. EE treatment was non-medical until he submitted to surgery in October.

Fall from ladders

EE was removing old lighting conduit, using a 10ft ladder. He was wearing fall protection, tied off to a structure in front of him, he could not tie off overhead. The conduit came out of its brackets, striking him on the chest and knocking him off of the ladder. The length of his lanyard allowed him to fall to the floor. He landed on his feet, spraining his left ankle.

Struck against a moving object

EE was driving a joy car which has two seats each seat facing different directions of travel. While swinging his body from one seat to the other he heard a pop on his right knee and felt some pain. EE did not seek medical attention until 5/27/2008. At that time Restrictions were issued.

Employee dumped load onto feeder, EE preparing to leave the feeder,power cable hung up between guard that keeps the cable away from the car.guard was hanging away from the car. employee stepped off the cable, cable reel took up slack as he stepped over the it.employee caught his foot on the cable fell to the ground.

While using a hand drill with a hole saw, employee was cutting a hole in a battery pack casing. The hole saw hung up, torque on the drill gun caused the drill to spin out of his hand. The drill struck the thumb area of his right hand, resulting in a break in the right wrist.

While unscrewing a light bulb, the bulb shattered, causing broken glass to cut the workers right wrist. The cut required three sutures and resulted in a work release with restricted duty.

EE was moving refinery bridge crane to an open area to work on hoist cable. As he was moving the bridge crane, he passed under a roof support beam. His left hand was on top of a control box, his hand became caught between the beam and control box, fracturing the tips of 3 fingers.

EE had just finished sitting down on the Tomcar and felt a discomfort on his lower back and as he proceeded to his work area the pain worsened.

Struck by falling object

Working on demolishing the interior ceiling in the reagent building from a work platform a 6""x8"" piece of the ceiling fell across the EE's right thumb,pinching it work platform rail and causing a laceration to the tip of the thumb.no medical treatment was required-treatment for infection was initiated on 5/21/2008

EE was walking to his jeep to get a tool that he needed and while doing so he stepped on an old track rail and twisted his right ankle. The jeep was located at the 6th level.

A Static end on Hoist Rope failed, causing the load block to fall on right hand. Lacerations were sustained on right ring finger of mechanic requiring sutures.

While placing slide gate bolt into position, the gate moved causing the bolt to spring up into the hand.

EE was making a belt splice in s810 and was using a utility knife to cut the belt, while doing so the blade slipped off the belt and went up, cutting his right hand wrist area, he recieved 4 stitches.

While Employee was pulling water hose out of ram car travel way the employee tripped on a pile of muck. In attempting to break the fall the employee stretched out her right arm landing on her right hand dislocating the little finger.
