Accident type, without injuries

Inspection of production shaft revealed a missing section of the west center guide rail about 8' above the bottom service landing. Damage guide rail interrupted operations for more than 30-minutes

Struck against stationary object

EE was performing preventive maint.@#276 belt Dr. #13 mainline, tripped on a joy pin anchor that was hidden by accumulation of muck in drive enclosure. EE was given 1st aide & returned to work. EE continued to work while under Dr. care w/unrestricted work activity. EE had a MRI & has seen a Ortho Specialist. EE had Surgery on 1/23/2012 to repair cartilage damage to his L. knee.

Struck against stationary object

EE struck head when entering the cab of Semi Tractor - Laceration required sutures - ordered off work

Over-exertion in lifting objects

EE experienced hernia while lifting a shuttle car tire to remove a chain. Treated as a first aid until surgery performed on 6/27/2011. Surgery was performed during EE scheduled days off.

Struck against stationary object

EE was ascending stairway of crystallizer building & struck the mid rail with right knee. conservative treatment resulted in return to normal duties. subsequent complaints resulted in MRI & surgery on 8/24/11 post surgery restrictions prevented return to scheduled shift on 8/27/11

Over-exertion in wielding or throwing objects

EE was tossing a coil of hose off his shoulder and lunged forward to clear the hose & felt pain in his abdomen -- also felt nauseated. Noticed a knot near his navel. Aggravated hernia.

Fall to the walkway or working surface

Employee fell and hurt L/shoulder on accident date. Continued to work regular job until 10/15/2011 Lost time started 10/17/12011

Struck by falling object

Truck driver was delivering tires to mine shaft #2, the tire struck an angle iron on lift gate. The tire deflected and struck driver causing him to feel a pinch in his lower back. Injury was not reported to employer until 9-6-11. He was having low back pain and was seen by doctor. The employee was treated as first aid until 9-26-2011 when he began losing time at work

Fall from scaffolds, walkways, platforms

Received medical notes on 09/27/11. EE was working in a pipe rack connecting steel when he lost his balance and fell, caught by his harness and lanyard.

Caught in, under or between a moving and a stationary object

We inadvertently didn't include this project as MSHA on quarterly 7000-2. As per MSHA directive, we have amended the 7000-2 for the 2nd quarter 2011 & are also filing an initial 7000-1 for this reportable injury. EE was rigging a beam. After it was raised, wind blew & the beam began swinging. EEs L hand was where the connection was to be made causing FX to L middle & ring fingers.

Caught in, under or between a moving and a stationary object

EE was removing old conduit with reciprocating saw. Blade caught on clamp & jerked the saw pinching Left #5 finger against conduit. Laceration had to be closed with sutures.

Received documentation 09/28/11. While setting a piece of pipe down on a flat bed, the tip of his left 3rd finger was caught between the pipe and flatbed, causing a fracture.

Employee removed PPE (glasses) for lunch and felt something in his left eye. He did not seek medical treatment until 9/1/11.

Over-exertion (Not Elsewhere Classified)

Cutting large rolled plate (20'X10'X1/4"") using a cutting torch to cut plate to fit through entry of Floc Tank, plate was sitting up right while being cut, after cut was complete plate shifted & started to fall forward, employee grabbed plate to stabilize & keep plate from falling against him, lowered to ground level with help and strained/wrenched his back in the process.

Contact with hot objects or substances

EE Stumbled on crack in concrete deck as he was retreating from spillage from a pump that was draining unexpectly - EE fell forward into hot brine burning both arms and (from splash) face and neck>

Flash burns (electric)

EE Burned from Arc Flash when cutting into live DC S.C. Cable for splice. EE locked out & Tagged out wrong circuit.

Employee was preparing caps for dryer. While preparing, stainless steel metal rolled causing a cut to his right thumb. Immediately after the incident, supervisor took precaution and medical treatment was followed.

Small left finger became caught between a cable tray and frame when a co-worker pushed tray into 45 degree angle, as instructed by injured employee.

Struck by flying object

EE was removing lug nut on Shuttle Car Wheel when the rim separated discharging air. EE was caught in the line of fire of the air discharge and debris - laceration to head required stitches.
