Employee was mopping tile floor in an office area, using a mop and mop bucket

Caught in, under or between a moving and a stationary object

While installing a 20'cable bolt, Ee's Left hand was guiding the cable bolt. Ee's glove was caught on cable and caused laceration and fractures to two fingers.

Ee was exchanging oxygen bottles and crushed the tip of Left Pinky Finger, causing damage to fingernail and slight fracture to very tip of pinky.

Struck by falling object

EE was clearing a dryer feed chute. Fine material from grating above made its way into their Right Eye and caused irritation

While closing a railcar bottom door, bar slipped and pinched Right ring Finger between bar and rail. Laceration required 3 stitches to close.

Struck against a moving object

Ee was riding service hoist when it came to an abrupt stop. Employee flexed knees and ankles to take shock. Resulting in fracture to metatarsal bone in left foot.
