Struck against a moving object

EE was operating shuttle car and hit a bump. EE bumped against inside of cab bruising L ribs and shoulder. EE was treated and released to perform normal work duties. Subsequent medical evaluation generated restrictions and resulted in lost-time 5/5/10.

Fall to the walkway or working surface

Employee was in process of moving continuous miner cable loops with a rope from long room on the left to cut ribs for survey sights when he slipped and fell down to the floor fracturing his right lower tibia and ankle.

Over-exertion (Not Elsewhere Classified)

EE reports that he slipped on mud at base of stairs near old floatation control room and tweeked his back. On 10/30 EE casually relayed possible back injury to non-WC med provider. Upon learning this, investigation conducted and comp accepted 11/6.

Struck by falling object

EE pulled pin to seperate hydraulic trailer from forklift and the tongue moved back falling down missing the blocks that were set to keep it up in the air. Tongue landed on his right foot.

Caught in, under or between a moving and a stationary object

Operator was tramming bolter and fell down, pulling bolter towards him and allegedly pinched his right knee between bolter and rib.

Struck against stationary object

EE was driving joy shuttle car to the miner and hit his head on roof top, causing him to fall out of the car and land on his shoulder. His first day of work under restrictions is Oct. 15, 2009

Absorption of radiations, caustics, toxic and noxious substances

Employee was in the process of shutting down the refinery when he stepped off the sump of the refinery. Proper PPE was being worn. The liquid in the sump came over his rubber boots and burned his foot.

Caught in, under or between a moving and a stationary object

EE placed himself between the bolter and tom car as he tried to move the bolter, while doing this, he pinched his right leg between both pieces of equipment.

Over-exertion in pulling or pushing objects

EE was pulling up about 30ft. of miner cable with cable hook and felt pain on her lower back plus she felt a sharp pain down her left leg.

A 3 foot mechanical bolt was being torqued by the mine crew. During the torqueing process the torqeuing wrench broke and part of it struck the miner who was standing about 6 feet away from the machinery.

A Maintenance worker was struck in the arm by a chip of steel as a co-worker was striking a bit block with a steel hammer.

Caught in, under or between a moving and a stationary object

Employee was in process of positioning a full 55 gallon drum of oil on a wooden pallet when his right ring finger was caught between another drum. The tip of the finger fractured and cutting it above the first joint. The employee received seven sutures to his right ring finger.

EE was climbing up side on a dump truck. The area around truck was muddy. As Employee ascended the step, EE's feet slipped and fell backwards off the step. The employee grabbed an I Beam to prevent the fall and Temporarily dislocated his R shoulder.

Fall from ladders

EE was descending a stable 8' stepladder set on flat concrete surface. EE reports thinking that he was stepping to ground level, but was one rung above. Fell backward striking L shoulder on ground. EE returned to work under medical restrictions.

Two EE's were removing the left cutter motor from miner 248. They had the motor loose and were walking it out with blue bars so they could put a chain around it and remove the motor. While walking it out the motor suddenly moved causing the bar to move rapidly, the point end of the bar struck his right thumb, causing a laceration on the inside of his right thumb.

Fall to the walkway or working surface

EE was walking to a safety meeting, walked into a drainage culvert and fell on her knee, causing the injury. The injury was minor, a contusion to the knee.

Air doors started closing when individual was half way through.

Absorption of radiations, caustics, toxic and noxious substances

While mixing Dexpan expansive grout, miner splashed some on his cheek. When he wiped his cheek off he accidently rubbed some into his eye. An infection was diagnosed on August 27, 2009 which made this a reportable injury at that time.

Contact with heat

EE was pulling wire, temperature was very high. EE came close to passing out, fell to the ground, unable to control muscles. Hospital determined he as dehydrated and had gotten heat exhaustion.
