Over-exertion (Not Elsewhere Classified)

Nov 2019, Ee was walking out of warehouse bay and stepped on uneven ground. Ee felt Left Knee pop, but did not fall. Feb 14, 2020...Ee had surgery on Left Knee as a result of injury.

Caught in, under or between a moving and a stationary object

Nov 5, 2019 Ee was using a bar to clean build-up off of a conveyor frame, the Ee's pinky was pinched between the bar and the handrail. Laceration was cleaned and employee returned to work. Recently, Ee noticed a growth under the skin. Dr recommended that it be removed. Ee had surgery on Feb 2, 2021. Ee lost time due to surgery.

Bodily reaction, (Not Elsewhere Classified)

Employee was exiting a Genco jeep and failed to clear leg before body out of vehicle. This caused a strain to employee's Left Knee.

Caught in, under or between a moving and a stationary object

Ee was driving a mantrip, and as traveling through an air curtain the Left front tire climbed the rib. Ee's Right knee was impacted and caused a fracture.

Over-exertion (Not Elsewhere Classified)

Employee was descending stairs and foot slipped on stair tread. Employee caught weight on handrail and strained shoulder. Employee was checked by a physician and returned to full duty and has been working without restrictions until today. Today the employee went out for shoulder surgery.

Struck by falling object

Ee was assisting in changing of hoist guide brackets. Some loose salt contacted the Ee's upper left back, just below the shoulder causing a fracture in 3 ribs. The fractures was not found by x-ray, but were discovered during an MRI.

EE was working in the auto shop, underneath a truck, when they felt something fall into Right Eye. EE rinsed the eye, but still felt symptoms. EE was seen at clinic, and Dr prescribed antibiotic for preventative measures and released to full duty.
