Fall onto or against objects

On a wet/rainy day, EE slipped while dismounting a dump truck. EE felt a pull in shoulders as they attempted to catch themselves using the grab irons. EE had not had any missed days or restricted duty until now. EE required surgery on Rt Shoulder as a result of incident.

Over-exertion (Not Elsewhere Classified)

Employee was returning to lab after collecting a sample. As ee stepped off of concrete stair landing to ground, ee felt a pain in back of Right Knee. After several weeks of discomfort, employee had MRI that showed a need for surgery.

Over-exertion in lifting objects

Trolley had slipped off of an I-beam. Employee reached across to lift trolley back onto I-beam and felt a pain in stomach. It was determined that the employee will need surgery to repair a minor hernia.

Struck by... (Not Elsewhere Classified)

Lining up a pipe flange when HDPE pipe laid over and struck shoulder and pushed employee into side of excavation. Pain in back showed itself several days later.

Caught in, under or between a moving and a stationary object

EE was cutting wire with a hack saw. After repositioning wire, EE misjudged hand placement and drew blade over Left Thumb causing a laceration that required stitches.

Struck by flying object

Employee stepped onto cage, and updraft blew debris up past employee's face. Something went behind safety glasses and went into Right Eye, causing minor irritation. Employee flushed eye several times. Later in the day, eye was still irritated, so we took employee to town. Dr. flushed the eye. Dr. gave prescription for antibiotic ointment for precautionary measure.

Caught in, under or between a moving and a stationary object

Employee was using a pry bar to unseat and slide out a tram motor. Motor moved, pry bar slipped, and employee's movement carried hands into the frame of the continuous miner frame. Left Middle Finger was pinched between pry bar and miner frame. 2mm was trimmed off the finger tip, 8 sutures were used to close the wound.
