While mining 2 left, top started working and the miner was pulled back to safe area. A fall occured measuring 50' X 20' X 8'. No injuries occurred. Timbered off and moved section to another area.
Roof fall in Return #1 Entry approximately 6050' outby face measuring 30' x 19' x 7'-8'. No injuries. Fall will be timbered across for a travelway.
The fall was discovered by mine foreman and state inspector while traveling the airways. The size of the fall was 40' x 20' x 4'. No injuries.
The roof was layered shell and fell above anchorage. The fall occurred in location of a hollow and close to outcrop. 45' x 20' x 5 1/2'. No injuries.
Fall occurred in the track entry at spad #112. Fall was 20'L x 20'W x 8'H. Intersection was bolted with 4' resin bolts. Fall was approx 3400' from face. Men were evacuated and area has been timbered. off. A cleanup plan has been posted and is being followed. There were no injuries.
Roof fall in #4 Entry 1 x-cut inby spad #7911. 55' x 19' x 5'. No injuries. Fall will not be cleaned up.

Roof fall in the entry at spad #3579. No injuries, passage was not affected. Fall was 20' x 20' x 8'. 3500' outby working section.
A roof fall occured at approximately 2:40 pm at White Queen Mine in the # 1 entry in-by spad #7790. The fall measured App. 4' to 6' thick and 90' long. Their were no injuries and no equiptment involved. Fall has been cribbed off and will not be cleaned up.
A roof fall occurred on # 2 section in # 3 entry at appromixately 4:50 PM. The roof fall measured 80' long 20' wide and 4' high. The area was supported with 48"" fully grouted resin bolts. There were no injuries. Fall was immediately timbered or cribbed off at all entrances.
A fall was discovered on 6/15/2006 in the #2 entry 2400' outby the active working section. The area was mined in Janurary 2006. 48"" resign bolts were being used with 6' torque tension bolts. Fall will not be cleaned up. All approaches are being timbered or cribbed. No passage of men was obstructed. No injuries reported. Fall was 23' long by 20'wide by7' high.
Roof Fall in #5 entry 1 crosscut inby Spad# 7150. Fall was 20' X 20' X 48"". No injuries occured.
Roof fall 130'X20'X5' in the #5 entry inby spad #172. No injuries, ventilation and passage were not effected. Roof support being used in area was 48"" resin bolts.
Roof fall in #2 entry 18' Long, 5' Wide, 42"" thick. No injuries were reported. Roof fall occured 640' Outby section, reported to Lincoln Selfe on 4/7/2006 at 5:20 PM. Fall will not be cleaned up. All approaches have been timbered off. Ventilation and passage of people not affected. Area was supported with 42"" fully grouted bolts with 16' J-channel.
Roof fall was discovered during weekly exam. Fall was 1 break inby spad #116, 2500' outby section. Size of fall was 60' X 15' X 8'. Fall did not disturb ventilation. Fall was dangered off from all approaches. No injuries were reported. Notified MSHA Asst. Dist. Manager at 10:15 AM about fall.
Roof fall was discovered across from spad 3630 near Horse Creek #3 belt. Fall was Approx. 13,000 ft outby the #1 section. Fall was at the Intersection of the #5 entry. Did not disturb Primary escapway. Fall was 40' long X 19' wide X 6' high. Area timbered off at all approaches.
Roof fall was discovered in 1st right off slip ridge mains in #1 entry near spad #6782 Fall measured 20' long 4' Wide X 4' High. 4ft. Fully Grouted Resin Bolts were being used in this area, fall will not be cleaned up. All approaches have been supported. No injuries occured. Fall was 12,800 ft from working section. Area Examined on a weekly basis.
Roof Fall was discovered in 1st right off of slip ridge mains in #2 entry near spad # 6710 fall measured 20'long X 4' wide X 4' High. 4 ft. Fully Grouted Resin Bolts were being usedin this area, fall will not be cleaned up. All approaches have been supported. No injuries occured. Fall was 13,200 ft from working section. Area Examined on a weekly basis.
On 1/27/2006 a roof fall occured on the #1 working section of white queen mine. the fall measured 90' long X 18' wide X 10' thick. no injuries were reported. The fall was cleaned up and re-bolted with 6' fully grouted bolts, 8' and 12' cable bolts.Additional bolts were installed on all sides of fall.
On the #1 panel off #5 Belt, near Spad 1263 between #1 and #4 entry on 1-26-2006 a roof fall was discovered. The fall measured 130' long X 20' wide X 6' Height. Fall occured in an out area. Area is only traveled on a weekly basis. 4' resin bolts were being used. Area was cribbed and timbered off. Ventilation was not disturbed.
On #1 panel off of #5 belt near spad # 1261 a roof fall was found on 1/26/2006. This area is traveled on a weekley exam. (Outby Area) No injuries occured and ventilation was not disturbed. Area dangered off and cribbed. Fall was 20' long X 20'wide X6' high.
FALL OCCURRED IN #6 ENTRY 3000 FT OUTBY FACE AREA. 48"" FULLY GROUTED RESIN BOLTS WERE BEING USED IN THIS AREA. FALL WAS 70' LONG, 20' WIDE, AND 6 TO 7' HIGH. FALL AREA WAS CRIBBED OFF AND NO INJURIES OCCURRED.
While attempting to start roof bolting, the 15' cut in #5 crosscut to the left, employee noticed that the right rib was loose. As he started to get a slate bar, the rib fell on his right foot, fracturing it. The rib was 18' long and 18"" to 5"" thick.
While sawing a timber for use in pillaring, EE was hit in the back with another timber that had been set when another EE bumped it as he was attempting to lay another timber down.
While Building a brattice in the x cut between #7 & #8 entry EE was working 18"" away from a check curtain. A 488 Long airdex scoop after stopping and sounding and his bell started through the curtain and hit him.
Member was setting 8' square 6 x 6 timbers. He knelt down to get a wedge when the timber he was setting fell and struck him in the back. He had not wedged the timber prior to turning it loose to pick up another wedge.
Member was carring 7' timbers to #3 entry to timber the roadway and felt pain in lower back. The accident was not reported to the safety department until 7/4/2006.
Employee was backing up an A.L. Lee fork lift to align it with a man door. As he was backing up he ran over a rock in the roadway causing the forklift to bounce into rib. He went to Raleigh General hospital where x-rays were negative. He was referred to a doctor.
Employee was hanging shuttle car cable in the #4 Entry. As he lifted it his knee gave out. He continued to work and later on during the shift he was carrying two 5 gal. cans of oil and his knee gave out again causing him to fall. He visited Raleigh General Hospital where X-rays were negative.
Was walking around feeder when his foot became stuck in the mud, causing him to twist knee. He visited Raleigh General Hospital where x-rays were negative. He was instructed to remain off work for 6 days and referred to Dr. Levins for an MRI. His first shift off due to injury was on 5-31-2006.
Employee was marking up belt hanger positions for a belt move while looking up at the top, EE stepped on rock causing him to fall landing on his side causing him to fracture his hip and a fracture to his femur.
Employee was tramming the scoop to retrieve supplies. The scoop stopped tramming, so he re-cycled it. When doing so the scoop jumped into tram hitting a hole in the roadway bouncing scoop in to rib. EE injured his back and right side. He visited Raleigh General Hospitial, Where he was instructed not to return to work until 5-15-2006.
There was a gob approximately 4"" deep on the offside of the belt drive. EE changed directions while walking and twisted his knee.
Employee was hooking a cable onto the scoop bucket as the scoop started to tram the scoop canopy caught on a roof bolt causing it to move to the right pinning him against the rib. He was transported to the hospital and released to return to work. On 3/30/06 He notified Safety that he had returned to the doctor an was told to remain off work.
While attempting to walk around the left S/C setting at the feeder employee stated that he struck his head on a piece of J-Channel that was hanging down 2"" from the roof. EE continued to work until 8:30PM. He transported himself to Rleigh General Hospital X-rays were negative. Attending Physcian gave EE a Doctors excuse to come back on 3/24/2006
EMPLOYEE WAS PULLING SLACK ON THE #2 SHUTTLE CAR FOR A POWER MOVE WHEN HE FELT A POP & PAIN IN HIS LEFT CALF.
Employee was entering parts building on surface. When entering he turned and twisted right knee. Investigation revealed that there was no stumbling or tripping hazards found.
On 2-22-2006 employee was getting into the Catipilliar IT-28. As he pushed off the ground he felt a pop in his right knee. He continued to work until 2-27-2006 and due to increased pain he took off work.
EE was attempting to pull slack on the feeder cable to allow the section electrician to connect it to the breaker. A rock Measuring 52"" long X 16"" wide X 7"" thick fell striking him on right ankle. He was then transported to CMAC General where x-rays showed a fractured ankle.
Employee started to bolt #3 face. He noticed a piece of rock leaning up against the rib. He attempted to flip it over. It became over balanced and mashed his right index finger against the rib causing an avusion to it. After visiting the hospital he was released for restricted duty.
Employee was climbing onto beam and slipped. While attempting to catch himself his right hand came in contact with a hold down with a sharp edge resulting in a deep cut to the palm of his hand requiring stitches.
Employee was picking up trash 2 crosscuts outby loading point when a bundle of plates fell out of the trash hitting him in the left hand requiring stitches.
While raising tailpiece, guard dropped smashing middle finger.
While employee was loading a tank of oxygen into the service scoop his foot slipped causing the oxygen to hit his left hand, fracturing his middle finger.
Member was removing drill steel ouf of hole after drilling. Rock fell and caught right hand between drill steel and rib. Received 12 stitches.
EE was attempting to pull a 1 1/2"" water hose through the hole in a stopping when the end suddenly came through and the coupling struck him in the mouth.
Member was taking discharge line off at pond pump. While jerking on line coupling broke loose and struck member in mouth upper lip requiring 7 stitches.
Member was helping cut down roof bolts from belt head hole that were bent. A bolt fell and struck member on wrist just above gloves, requiring 3 stitches. Marfork safety did not know about accident until 7/10/2006 due to miner's vacation from 6/30/06 - 07/9/06.
Member was replacing rubber in stacker tail piece while attempting to cut it, the razor knife slipped cutting his left wrist. He received 3 stitches.
Employee was attempting to disasemble 2 joints of 4"" plastic water line as it came apart a joint of it swung hitting him on the nose.
Member was holding a primary belt scraper while it was being cut with a reciprocating saw. The scraper started vibrating and he moved his hand closer to the saw blade when doing so the saw blade jumped out of the cut and punctured his hand between his little finger and ring finger at the knuckle. 3 stitches were required to treat the injury.
Member was cutting belt in order to make a splice for a belt move. While cutting the belt, the knife slipped cutting his knee. Member recieved 11 stitches to his right knee.
Setting a timber at the #8 Seal , employee was hammering a wedge in place. He missed the wedge striking his left ring finger with a hammer causing a fracture.
Employee was trimming a belt splice when his knife slipped cutting his knee. He completed his shift, then sought medical attention where he then received 6 stitches to his left knee.
Ee was bolting in the #6 entry. He lowered the drill head and the drill steel fell out of hole. Ee twisted around to grab it and felt intense pain in both knees. He was transported and admitted to Raleigh General Hospital due to pain and swelling.
On May 20, 2006 due to degenerative changes caused by repeated trauma over a period of several years, employee was diagnosed by medical records to have degenerative conditions causing severe back pain.
