Employee was driving F350 company truck when the truck started to lose power and smoke coming from underneath front end of vehicle. Employee pulled vehicle over and saw flames and immediately started to put out fire; called Whitesville Fire Department to assist.

Employee placed his hand on rail for leverage to stand up when rail slid over and smashed his right pinky finger against another rail resulting in (distal) amputation by physician of right pinky finger below first knuckle.
As Employee prepared to operate locomotive he put his finger between frame of motor and shield. As he sat down he placed his bodyweight on shield smashing his right middle finger. The accident was not reported to Marfork Coal by the injured. An internal investigation was conducted and accident did occur on the job. On 11-22-13 finger was amputated due to severe infection.
Employee was pulling a timber across the track when he felt a pull in his groin.
While Employee lifted lid to the shuttle car cable reel the SC operator pulled forward causing the lid to fall smashing Employee's left ring finger. Employee was transported to Raleigh General x-rays were taken and no broken bones. Employee was scheduled for surgery at CAMC Memorial Division for skin grafts to repair the finger.
Employee was hauling supplies to the section. Employee crossed the track with scoop causing his head to hit canopy jamming his neck.
Employee was bending over to attach fish plates to track rail when he experienced pain in right side of neck and shoulder. Employee was treated at Raleigh General Hospital. Employee continued working until 11/20/2013, at that time the physician instructed him to be off work until further notice.
Employee was carrying a bucket of miner bits when he felt a pull in his back. Employee tried stretching his back to get some comfort and when he did he felt a pop. He immediately came outside and was transported by EMS to Raleigh General Hospital for treatment.
As Employee walked past scoop batteries his shoulder hit the lid which was propped up. The lid fell striking Employee's right hand which caused a simple fracture to the 5th metacarpal bone.
Employee picked up a 40lb bag of rock dust to put in duster. While picking up bag employee felt sharp pain in lower back. Employee did not seek medical treatment until August 12, 2013 where he has undergone and will continue physical therapy for two weeks.
Employee was removing a slide skid from a flat car that was hooked to the motor. When removed the flat car moved backwards from the slack in the draw head causing it to flip downward which smashed Employees index finger on his right hand.
As Employee was traveling on main haul road (on his way home) he fell asleep causing him to wreck his truck. He received minor injuries to his upper body.
Contractor slipped and fell on a piece of belt causing the right knee to fold behind him. Contractor was treated at CAMC General Division where he was diagnosed with cartilage damage.
He was driving shuttle car from the continuous miner toward feeder, attempted to readjust himself and tires hit hole in roadway causing him to hit canopy with hardhat jamming his back. He continued working 2 weeks, went on vacation for 1 week and began missing work on 7-8-2013. Company conducted investigation to confirm validity and found it valid on 7-18-2013
As Employee's were transporting conveyor pot to the continuous miner. The conveyor pot was chained to the forklift. The chain broke causing the conveyor pot to fall striking Employee on the right foot. Employee was transported to Raleigh General Hospital for treatment and was diagnosed a #2 and #3 metatarsal bone fractures.
As Employee was crossing over belt he stepped down placing his foot on a rock causing his ankle to twist. Employee was transported by EMS to Raleigh General Hospital for evaluation.
As Contractor was loading belt structure when a bottom roller fell off and hit his foot.
As Employee was pushing a loaded wheelbarrow it started to turn over. Employee attempted to counter balance the load when he strained his back.
Employee was unloading belt structure from scoop. The top roller was stuck and as Employee pulled on roller he felt a pain in lower back left leg.

As Employee was horse playing with another employee his finger got caught on the belt strap of the other employee causing a fracture to his pinky finger.
Employee was skiving belt when the utility knife he was using slipped and cut his left hand. He was treated at Raleigh General Hospital where he received six (6) sutures.
As Employee was taking down a stopping he picked up a mandoor and a jack fell over pinching his hand between the jack and mandoor. Employee received treatment at Raleigh General Hospital where he received three (3) sutures.
Employee was tightening lug nuts on the #1 DBT scoop when his ratchet slipped striking Employee on the chin and broke his front tooth.
Employee was cutting off a belt flapper and attempting to remove a damaged clip with channel locks. The channel locks slipped striking Employee in the mouth breaking a tooth. Employee was seen by a dentist and had his tooth repaired.
Employee was cutting the belt for the angle of the vulcanizing splice. The knife slipped off the belt striking Employee on the left hand cutting through his gloves. Employee was treated at Raleigh General Hospital where he received 4 sutures left index finger and 2 sutures left hand.
Employee was helping close doors on railroad cars and while putting pressure on bar to latch door the bar slipped out of the latch causing Employee to fall backwards injuring his lower back. Employee was treated at Raleigh General Hospital and the CT Scan was negative and received a full release. Management was notified Feb 20,2014 he would begin physical therapy.
As Employee was moving Feeder in outby area of the scoop roadway the Feeder was stuck in a water hole. Employee placed a crib block under the cat to free up Feeder. As the cats were being raised Employees right ring finger was pinched between the crib and the lifting jack. Employee received treatment at CAMC General Division where he received 13 sutures.
As Employee was bolting a piece of drill steel broke off striking him on the forehead causing a laceration. Employee was treated at Beckley Appalachian Regional Hospital and received four (4) sutures.
Employee was cutting two (2) 4"" to 6"" pieces of belt. Employee was flipping belt over and making last cut when the utility knife slipped causing a laceration to right thigh. Employee was treated at Beckley Appalachian Regional Hospital where he received 6 staples.
Employee was helping put a piece of metal on a tail piece, building a rock box. Employee was holding metal when the metal slid falling on right foot. Employee went to CAMC where he received treatment and xrays were negative. Employee was not notified of his fracture until March 29, 2013.
As Employee was cleaning out the tripper chute on the plant clean coal chute while using a 10ft piece of conduit; the material broke loose causing the conduit to slip Employee pulled on the conduit and felt sharp pain in upper back and right shoulder. Employee was treated at Raleigh General Hospital and received a full release. Employee started physical therapy on 4/29/2013.
Contractor was walking by the #3 Fairchild Scoop and lost his footing when he tripped and hit his chin on the canopy of the scoop. He was treated at Raleigh General Hospital where he received two (2) sutures.
Employee was drilling a test hole in #7 right when a piece of steel fell and struck Employee in the mouth causing a small laceration and chipped two teeth.
Employee was hanging continuous miner cable when he felt a slight pain in his shoulder. The Safety Department was notified April 1, 2013 Employee was receiving physical therapy for his injury.
Employee was loading 60lb rail off of a forklift onto the rail carrier. Forklift would not raise high enough to clear the standards on rail carrier due to height. Employee was lifting one end of rail at a time when rail fell smashing his left ring finger. Employee was treated at Raleigh General and was diagnosed with a slight fracture and received six (6) sutures.
Employee was lifting a blower motor off the ground to put in the parts building. As Employee lifted the motor, the motor shifted causing injury to right thumb. Employee was treated at MedExpress and was diagnosed with a sprain/strain to right thumb and given an immobilization splint.
While prying the cable out from around the cable reel on # 10 shuttle car sprocket the pry bar slipped and caused right pinky finger to have a laceration requiring 4 sutures on the tip of finger. the distal phalanges to have a hairline fracture.
