Accident type, without injuries

A non-injury roof fall was discovered on the No. 4 Belt between 2 and 3 cross cut in the belt entry located approximately 36 feet outby spad No. 3824. Fall measured approximately 15 feet long x 12 feet wide x 4 feet high.

Accident type, without injuries

Non-injury roof fall occurred on the No. 3 Section in the No. 5 Entry 1 break outby spad No. 4283. Fall measured approximately 30 feet long x 18 feet wide x 8 feet high.

Struck by falling object

While resetting timbers, a rock measuring approximately 69 inches x 50 inches wide X 6 inches thick fell from roof striking employee on the right foot. Resulted in fractures to the foot.

Over-exertion (Not Elsewhere Classified)

While operating the continuous miner in the No. 1 Heading, the equipment operator was handling the trailing cable and slipped, spraining the left knee.

Fall to the walkway or working surface

Employee was coming down embankment, slipped on loose dirt and twisted right ankle.

Caught in, under or between a moving and a stationary object

Employee was dropping grating around the power center and left thumb got hung in the grating. Resulted in a fracture to the left thumb.

Over-exertion in lifting objects

While hanging trailing cables from continuous miner and roof bolter, employee felt a pop in left knee.

Struck by falling object

While changing scoop batteries, a piece of rib rock measuring 47 inches x 13 inches x 6 inches fell on miner's right leg.

Struck against stationary object

While being transported to section on rail mounted vehicle, employee's knee was protruding outside the confines of the passenger compartment and came in contact with a timber set alongside the belt line.

Struck by falling object

Employee was on knees loading supplies onto the bolt machine. Roof rock feel out from between bolts striking employee on back of calf.

Employee was involved in horseplay which caused injury to the lower back.

While bolting in the No. 5 Entry, on the No. 3 Section, rock fell from mine roof pinning the miner's leg against the frame of the DDR bolter. Resulted in fractures to the lower leg.

Employee was prepping to shear the fingers of a finger splice. EE started to shear the fingers with an oscillating tool and slipped with the shears causing the shears to strike right pinky finger.

Employee was setting a Heitzman jack when a piece of roof rock fell measuring 3 feet x 2 feet x 8 inches thick. The rock hit employee's right foot.

Employee was installing a chute on the back of a concrete truck and the chute came down on EE's middle finger cutting the tip of the finger.

Rib rolled out beside of track. Miner was riding personnel carrier with feet on edge of frame. Rib roll caught left foot between rock and frame resulting in a fracture to several metatarsal bones in left foot.

Employee was welding a piece of metal on a sled when the tractor trailer started backing up and ran over employees right leg

Employee was handling a piece of sheet metal 5 feet x 10 feet x 1/4 inch thick and strained their back.

While nailing curtain to rib, some material hit the injured in the right eye over top of EE's safety glasses.

While installing roof bolts, rock fell from mine roof striking ATRS and then struck the injured on the left shoulder, sliding down injured back and striking right foot.

Roof bolter operator was installing roof bolts. Operator was installing a roof bolt into the chuck of the machine when hand slipped and hit the controls causing the chuck to rotate entangling operator's hand. Resulted in fractures and lacerations of the left hand and ring finger.

Employee was carrying materials up and down a ladder. Employee missed the 2nd riser from the ground and fell backwards. Employee's left foot got caught in between risers and twisted it.

In the process of dismantling a scaffold an 8' piece of lightweight aluminum scaffold support beam fell a distance of about 5' striking the employee of the right shoulder.

While attempting to remove plywood contractor employee was hit in the head with a small steel beam that was lying on the plywood.

Employee was squaring the end of the move belt rubber in preparation for splicing. While using a utility razor knife, the knife slipped causing a laceration to employee's left hand. Resulted in four (4) stitches.

While observing the plant clean coal belt, employee stepped on a pallet and rolled ankle resulting in a small fracture.

Miner tripped and fell into the continuous mining machine striking face and mouth against the machine. Resulted in a fractured tooth which was repaired.

While loading a shuttle car conveyor chain onto a rail mounted flat car, a portion of the conveyor chain slid off the car, striking employees ankle and foot. Resulted in a fracture.

Employee was cutting tag off a pair of new boots when knife slipped causing a laceration to left wrist. Resulted in eight (8) stitches.

Smashed finger between rock box and slate bar. Resulted in five (5) stitches.

Employee smashed right ring finger between door and frame on D-10 Dozer. Resulted in a fracture to the tip of the finger.

Employee was handling tin. Resulted in a laceration requiring three (3) stitches.

Was washing down floor. There was a small pile of magnetite in floor. Tripped over pile and tried to catch self. Little finger on left hand went into (through) a fan guard. Cut the end of finger off through the gloves.

Employee was on surface loading rock dust into a track mounted dusting machine. While disconnecting the discharge hose from the machine, the connector struck EE in the face/nose area. Received two (2) stitches.

While operating roof bolting machine, miner struck a roof bolt plate lying in the supply tray with left forearm. Resulted in a laceration requiring 5 stitches.

Employee was removing lid from a metal oil can and cut wrist. Resulted in seven (7) sutures.

Struck by flying object

Employee was working near the section conveyor tailpiece, when a rock came through the tail piece knocking the guard loose striking the employee causing a laceration around left eye. Resulted in 16 sutures.

Employee had hand on canopy of underground forklift while operating. Canopy contacted mine rib pinching left hand. Resulted in a laceration requiring 13 sutures.

Fall onto or against objects

While accessing operator's deck of a scoop, employee came in contact with a weld burr lacerating left arm resulting in five (5) sutures.

Crew was departing section on #4 MAC personnel carrier when a piece of draw of shale rock fell from mine roof striking employee on hard hat causing a laceration to EE's forehead. Required 6 sutures.

While using a set of chain ratchets to position the tailpiece during a belt move, the chain ratchet released from the anchor point striking the injured in the mouth dislodging front upper tooth.

Employee was handloading steel I-beams by hand onto a rail mounted flat car smashing left middle finger. Resulted in a laceration requiring 4 sutures.
