Fall was in the neutral entry between 2-3. Spad 6379. Fall was 40' L - 20' W - 8' to 9' H. It was bolted with 6' torque tension fully grouted bolts, developed 4-15-07. Area will not be cleaned up and no injuries were recorded. Airways and escapeways are not affected.

Fall was located 1 break outby spad 6297 in the #5 neutral entry approx 2800 ft from face. Fall measured 20'L x 20'W x 6 1/2 - 7'H. The area was bolted with 6' torque tension bolts. No clean up plan has been posted. No plans to clean it up. No injuries or airways blocked.
Roof fall in #5 entry - last open crosscut. 20' x 10' x 8'. No injuries. A roof bolting machine was blocked.

Due to the Joy 14-15 Continuous Miner loosing power in the last lift of a pillar block in the #2 entry, a roof fall (40' X 40' X 5'-7') fell on it. No injuries occurred and the Continuous Miner was recovered.
Fall was measured at 20' length, 20' width, 6 1/2' height. The area was bolted with 6' torque tension bolts. Area was breakered off using timbers and the intake was rerouted. Near spad #6279 2600' from face. No injuries reported, no restrictions of air.

A fall was discovered in the intake cushing at a stopping, which will be rebuilt. No injuries and the fall will not be cleaned up.
While mining through a barrier block water was cut into on the cut through. No injuries occurred.
A fall was discovered measuring 60'x30'x6' by the mine foreman and state inspector at ss7890. There were no injuries and it will not be cleaned up.
A fall was discovered measuring 20' x 20' x 8'. No injures occurred as a result. The fall will not be cleaned.
A roof fall was discovered 800' outby current face measuring 70' x 20' x 6 1/2'. No injuries occurred as a result. The fall will not be cleaned up.

Individual was attempting to pull a matrip to motor barn with another mantrip. The mantrip being towed derailed in a curve and landed on his right foot resulting in fractures. Individual was communicating with operator of the other mantrip while standing beside the mantrip.
While replacing spring pedestals in the #1 refuse dewatering screen, ee's feet slipped causing a strain to his abdomen. Marfork Safety was notified that he did not return until 11/19/07 on 11/12/07.
EE stated that while moving the miner cable over, he felt a pain in his back on 10/15/07. On 10/17/07, when he did not report to work and attempts were made to contact him and were not successful. On 10/22/07 EE called and said he was off due to injury.
When employee turned to exit the deck of an Eimco scoop he felt his right knee pop. He continued to work until 8-30-07.
While walking through the cross sect. from #5 to #4 entry, a piece of rock measuring 6' x 2'6"" x 3""-4"" fell striking EE on the head & back. X-rays were taken with the results being negative.
Employee was making a flexco belt splice with a co-worker. As he reached for the nail plunger, the co-worker accidentally hit his hand with the hammer.
While roof bolting the drill steel became stopped. Harper towered the drill boom to check the selction, when he raised and back up. He placed his hand on top of the drill pot causing it to be pushed into the drill steel
While working on a 55 KVA power box, EE came in contact with high voltage, when he raised the top lid.
Employee stated that on 6-21-07 when attempting to hang a miner cable he felt a pain in his back. This accident was not reported until 8/27/07. Employee continued to work until 8-30-07 when instructed to remain off work.
Member was injured while working on a repair of a Joy 21 shuttle car. The shuttle car was in a raised position on top of crib blocks when the cribs shifted causing the shuttle car to fall on him.
A piece of rock measuring 15"" x 16"" x 1"" fell from the roof striking him on the shoulder. He was transported to the hospital; not given a diagnosis. On 6/21/07 after having a MRI he was diagnosed with a fractured clavicle.
EE was helping another move 2 tool sleds with Fairchild 35C scoop & a chain. Scoop operator trammed forward; a hook located on the side of the bucket caught on 3rd sled causing it to slide forward pinching both of injured EE's feet.
While tramming a continuous miner employee attempted to place a loop of cable slack onto the conveyor boom without deactivating the remote control. He accidently hit the conveyor swing pinning himself against the coal rib.
Individual came outside after shift and remembered he had to deliver a contractor back to the section for the miner. When returning to section he went to the miner and bent down to look into the panel. When the contractor went phase to phase causing an arc flash burning his face and hands.
While riding the mantrip a rock fell and hit ee in the ribs. It took about 10 feet before the mantrip could stop. When ee exited the mantrip he complained about his ribs hurting.
While working on a dozer starter EE lost his grip causing the starter to fall on his right knee. This is only 1 of the 3 traumas that has occurred to his right knee while working at Marfork. He continued to work until 7-24-07.
Employee was in the process of lifting the back plate pulley assembly to the take-up unit at 4 head with a 3/4 ton Harrington chain hoist. The hook rigging failed, allowing the assembly to fall on his right foot, causing a fracture of his 7th metatarsal bone.
Employee was moving the continuous miner cable from the roadway when he twisted his right knee. He was seen at the health place on 1-8-07 and was sent for an MRI. On 1-17-07 the results of the MRI were: a torn meniscus.
While attempting to open the blade switch on a new KUA power center located along the #6 belt line, causing a laceration requiring 9 stitches.
While pushing coal in the steel area with a D7 dozer, ee ran over a hump causing his head to hit trac box.
Employee attempted to exit a mantrip when his foot slipped causing him to put his hand down on the ground causing a laceration requiring 6 stitches.
While operating a scoop, member had his hand on outside of canopy causing his hand to get mashed between the rib & canopy, causing a laceration that required stitches.
While trying to remove a piece of nylon band from a tail roller, it caused a laceration requiring 9 stitches on the left little finger.
Bent over to pick up gloves, hit left elbow on a roof bolt plate causing a laceration that required 4 stitches.
Cutting piece of 1/2"" conveyor belt which was located on spool. Knife slipped cutting the index finger of left hand. Received 5 stitches.
While attempting to cut tape off a lever of a Joy 21 shuttle, the knife slipped causing a laceration to his ring finger of his left hand. The laceration required 3 stitches.
While handling belt structure, EE's right middle finger became pinched between the belt tail piece and structure.
While attempting to let the roof bolter cable down that was hanging up in the #6 entry, the reel was activated causing employee's right hand to be smacked against the roof, lacerating the index & middle fingers.
While loading belt structure, employee's hand became pinched between two pieces of structure.
EE was installing a cable bolt in intersection of #3 entry at spad #6273. While pushing the bolt up, EE started to rotate bolt. After pushing bolt up approx 5', the bolt became frail and bowed out while spinning, striking EE on the left arm. Diagnosed with fracture at RGH Hospital. EE has a cast on his arm.
Individual was using a slate bar to turn roll on rock sizer when the slate bar broke causing him to fall on left leg resulting in a fracture.
Individual was tramming bolter for new set up and ran over 8"" ledge causing him to hit his head and ear on canopy resulting in a laceration requiring 7 stitches to ear.
Employee was placing bolt in the hole to be pushed up by the roof bolt machine when a piece of rock fell from the top and struck him on the ear causing a laceration. Height was approx. 60"". He received 8 stitches.
Individual was trimming a piece of belt and knife slipped resulting in a laceration to left forearm requiring 10 stitches. Individual didn't miss any work.
EE hit his head on the roof and jammed his neck down into his shoulder. This has happend a few times before. It hurt for a little while then quit.
Member was walking across section and came into contact with metal strap that had been bent down on one end by either boom of miner or canopy of equipment resulting in a laceration to left shoulder requiring 11 stitches.
Drilling hole when a piece of draw rock fell out drill out throwing a piece of rock into mouth hitting tooth. On 1/31/07 the safety dept was notified that this individual had sought dental repair.
While cutting belt rubber to make a splice, member cut his right leg above the knee. The injury required 3 stitches.
Due to repeated trauma, employee has been diagnosed with severe degenerative arthritis of the spine due to occupational illness. This was not due to a single event and is substantiated by medical records.
Miner was diagnosed with pneumoconiosis and excercised his right to be moved to an atmosphere with not more than 1.0 concentration of dust.
