Accident type, without injuries

Approximately 1:10am, a roof fall occurred on the #1 Section in #8 entry at 12cc 1cc inby spad #4379 trapping 2 Mobile Roof Supports (MRS's) in the fallen material. There were no personal injuries.

Struck by falling object

Employee was setting timbers for the next retreat mining lift inby spad 21417 when the slab cut entry that was previously cut fell to the roof bolts causing draw rock to strike the employee. Employee was coherent and alert while being transported outside. During transportation to medical facility they suffered a cardiac arrest. Incident remains under investigation.

Struck by flying object

Employee was working on the RCM replacing core breaker and wear shoes while putting the pin through the shoe and core breaker with a hammer something came off hitting their left eye.

Struck by falling object

EE was removing drill steel from a drilled hole. Pusher and extension separated in hole. Pusher stayed in drill chuck, extension stayed in roof. Extension fell out mashing left pinky finger between pusher and extension causing a tuft fracture to the end of the left pinky finger.

Inhalation of radiations, caustics, toxic and noxious substances

The Operator was notified on 04/16/2026 that the employee has been diagnosed with Occupational Pneumoconiosis.

Employee was diagnosed with Carpal Tunnel Syndrome
