An unplanned roof fall has occurred at Short Sub Mains #5 entry spad 3067. The roof fall was approximately 30' x 40' x 4'. No injuries or equipment involved.

An unplanned roof fall has occurred at Horse Creek #4 spad 4022. The roof fall was approximately 40' x 30' x 5'-7'. This area is a weekly travel way. No injuries were report and no equipment was involved.

An unplanned roof fall has occurred on #2 Section at crosscut #3 to #4 spad 5876. The roof fall was approximately 65' x 20' x 6'. No injuries were report and no equipment was involved. A revision to the roof control plan has been submitted to the District Manager for approval.

An unplanned roof fall has occurred at Parker Mains just inby 110 spad in the intake #7 Entry outby seal area. The roof fall was approximately 180' x 18' x 7'. No injuries were reported and no equipment was involved.

An unplanned roof fall has occurred at the intake #6 entry at spad 262. The roof fall was approximately 60ft x 20ft x 6ft. No injuries were reported and no equipment was involved. Lifeline was rerouted and the miners were made aware of the change.

An unplanned roof fall has occurred on the #1 Section #6 entry 1 Belt at spad 242. The roof fall was approximately 30'x19'x5'. No injuries were reported or no equipment was involved.

An unplanned roof fall occurred at survey spad 3106 #3 entry inby dumping point. The roof fall was approx. 40'x18'x7'. The initial 103(j) Order was modified to a 103(k) Order. Both Federal and State agencies approved a clean up plan to rebolt and clean roof fall. On June 17 the 103(k) Order was lifted to resume normal production. No injuries was reported or no equipment involved.

An unplanned roof fall has occurred at spad #3916 on the Arabian Mains #6 entry. The roof fall was 20' x 20' x 5'. No injuries involved, no equipment involved or passage blocked.

An unplanned roof fall has occurred on the #1 Section #4 Right Return panel spad 4553. The roof fall was 20' x 19' x 4'. No injuries were report or no equipment was involved.

Caught in, under or between a moving and a stationary object

Member was in the deck operating the bolter. He went to reposition himself and stuck his hand on top of the canopy to pull himself up and hit the tram. The bolter moved, smashing his right hand into the top. Member immediately ceased to work and was taken by ambulance to CAMC General where he was told his right hand/middle finger had to be amputated.

Employee was installing a clutch assemble and while holding it in his hand it slipped out and fell on his right hand.

As employee was checking faces he stepped on a ledge in an intersection causing his ankle to roll. Employee was transported to CAMC General Hospital for treatment. Employee was diagnosed with a fracture to his ankle.

Employee was walking across the bench area, stepped on a loose rock slipped and fell. Resulting in a hairline fracture of the right fibula

Struck against stationary object

Employee was walking outside to get welding supplies. As he was walking under the canopy he struck his head on an 8in I-Beam causing swelling to his face. Employee was treated at the scene by EMTs and transported to Raleigh General Hospital for further evaluation.

Struck by falling object

While Employee was bolting in #3 entry a loose piece of draw rock fell striking the ATRS on the roof bolter. A small piece of draw rock struck Employee on the left thigh. Employee was transported by EMS to Raleigh General Hospital. X-rays were negative and was instructed to follow-up with a specialist.

Over-exertion in pulling or pushing objects

As Employee was attempting to pull water manifold out of gob he felt a sharp pain in his left shoulder and back area. Employee drove to Raleigh General Hospital for evaluation.

As Employee was moving a battery lid the lid slipped and fell striking his right foot. Employee was treated at Fayette Foot and Ankle Clinic. Employee had xrays taken which were negative.

Caught in, under or between running or meshing objects

As Employee was bolting in #7 entry the drill chuck was not lined up with the bolt head. Employee hit the rotation lever causing his hand to twist in the bolt. Employee was treated at the scene by EMTs and drove himself to Raleigh General Hospital for further evaluation.

Struck by falling object

A rock measuring 4' x 7' x 1""-4"" fell striking Employee on his right foot. Employee was transported to Raleigh General Hospital and was diagnosed with a fractured big toe. Employee was scheduled for out patient surgery but instead was admitted on 9-23-11 for surgery to correct his fractured toe.

Struck against stationary object

Injured worker was stepping across to a beam. A rock rolled under his grounded foot, causing his leg to bend the wrong way and pop.

While employee was tightening bolts on the Plant Feed Belt he felt a sharp pain on his left side. Employee was elevated and tied off with his safety harness. He was using his right hand to hold ratchet and left hand holding onto belt structure. Employee was treated at Raleigh General Hospital and was diagnosed with bruised ribs.

While Employee was raising the drill pod of the roof bolter, to push bolt up, a piece of draw rock fell striking employee on his hard hat. Employee was treated at Raleigh General Hospital and x-rays were taken.

Struck against stationary object

As Employee was pulling curtain out of gob he stepped on a rock causing his right knee to twist. Employee went to his family care physician the following day 8-3-11 where xrays were ordered for 8-4-11.

While employee was picking up water line to put a fire valve in his back struck a pie pan which had been bent up causing him to fall. The Safety Department was not notified employee missed work or was seen by a physician until 8-8-2011.

While employee was building a stopping on #2 Section in #5 entry he injured his left little finger. Employee was treated at Plateau Medical Center and was diagnosed with a small fracture and also hyper-extended the growth plate.

As Employee was cutting timbers he slipped and fell striking his left knee on the track. The area he was cutting timbers was wet and muddy. Employee was treated at Raleigh General Hospital where he was diagnosed with a left knee strain.

As employee was walking around mantrip he slipped and fell. Employee landed on the ground causing his shoulder to dislocate. He was transported to Raleigh General Hospital and was diagnosed with a shoulder dislocation and a small fracture. Medical staff set his shoulder back in place and referred employee to a specialist for further evaluation.

Rubbed or abraded

Employee was conducting a pre shift examination on Horse Creek 9 Belt at 76 break. Employee crossed under the belt at the cross under. As employee bent down he felt a sharp pain in the right knee which started to swell. Employee drove to Raleigh General Hospital for treatment. X-rays were taken, he was diagnosed with a sprain knee and to follow up with an orthopedist.

Employee was cleaning 5 entry with a scoop. As he was cleaning the right rib piece of line curtain fell. He went to re-hang the line curtain a piece of rock fell out of the rib pinned right foot. The rock was approx 72x32x16. He was transported to CAMC and was diagnosed with 6 fractures in his right foot. He was given instructions to follow up with an orthopedic surgeon.

Over-exertion in lifting objects

Employee was in #3 entry hanging miner cable. As he was hanging the cable he felt pain in his back. Employee was transported to Raleigh General Hospital and was diagnosed with a back strain.

Struck by falling object

Employee was building a stopping in #6R across from feeder. A piece of rock broke loose from the rib striking employee on the right shoulder and right hand. Employee was transported to Raleigh General Hospital. Xrays were taken and was told to follow up with a specialist for further evaluation.

Struck against a moving object

Employee was operating a Mack Coal truck when the truck struck a pot hole. The air ride seat lost air causing the employees seat to bottom out, injuring his lower back. The employee was placed off work due to a diagnosis of bulging disk's.

Over-exertion in lifting objects

Employee was hanging cables at the right dump on the Feeder line. Employee felt a sharp pain in his back. Employee refused ambulance and drove to Raleigh General Hospital. Employee was diagnosed with a strain and was instructed to remain off work for five (5) days. Employee had assistance hanging cables.

Struck by falling object

Employee was bolting a corner bolt in #2lt. Employee was standing in a walk through bolter when a piece of rock fell striking employee on the right hand. Employee was treated at Raleigh General Hospital and was diagnosed with a strain.

Over-exertion in lifting objects

Employee was lifting a shield on the #2 Motor to check the wires and plugs on the blue box. As he was lifting the shield he felt a stinging sensation in his stomach. He then noticed a large knot raise up on his stomach. Employee immediately ceased work and drove himself to a medical facility which he was diagnosed with a hernia. Employee continued working until 5-13-11.

Employee was getting out of a personnel carrier when his arm slipped, causing him to fall to the ground approximately 30 inches straining his left arm and right leg. Employee drove himself to Appalachian Regional Hospital for treatment.

While Employee was moving a roof jack it fell striking employee on the right foot and leg.

Member was standing on 1' x 2' platform raising flop gate at head of reclaim belt when he lost his footing and slipped on the platform. Rain and slick conditions caused member to slip off of the top step and bump his shin and knee. Member saw a doctor where he was told to remain off work for a few days due to excessive swelling on his knee and fluid building up around the area.

Member was pulling a water line with the scoop and got his right hand tangled up in the loop of the water line. The member immediately ceased to work and co-worker performed first aid on him in the nature of wrapping his hand then he transported himself to Raleigh General where he was then diagnosed with a broken hand.

Member was setting bits on the left continuous miner and smashed his ring finger with a hammer which cut his finger and cause his fingernail to fall off. Member immediately ceased work and was transported by paramedics to the hospital. At Raleigh General Hospital he was told to remain off work in order to avoid the risk of infection.

Employee was working on the #2 clean coal screen. After completing the repair work to the screen deck, Employee was climbing across the mechanism when his arm slipped out from under him. Employee landed on the screen on his back and felt pain in his neck. Employee was seen at Raleigh General Hospital on 3/6/11 and xrays were taken. He is to follow up with a specialist on 3/16/11.

Absorption of radiations, caustics, toxic and noxious substances

Contractor was told to plaster stoppings but had left his plastering gloves on the mantrip, which had already returned outside, so his boss informed him to plaster the stoppings with metacarpal gloves instead. Contractor later went to Raleigh General seeking medical attention, where he was told he had sustained first and second degree chemical burns to his hands and arms.

Member was dropping cable, the reel took up on the roof bolter and the cable hit him in the right wrist. Member did not immediately cease to work and got a sling to use for the remainder of his shift. When his shift ended, he drove himself to Med-Express and received medical attention, where he was instructed to take off and rest his injury a few days.

Member walked up and asked the miner man how much more he lacked so he could come across and clean that side of the section and the cut where the miner was fell in on the miner and a piece of rock slid down and hit member on the ankle. He was taken by ambulance to Raleigh General where he met Analabs for a drug screen and was scheduled for surgery on 1/29/2011.

An EE was running the scoop and scooped up the pressure pump in order to move it. The door on the scoop deck was not shut and injured EE went to shut the door. Other EE thought that IE had completely moved out of the way of the scoop, but he had not. Other EE began backing up the scoop bucket and hit IE and pinned him against the right rib. He sustained injuries to his knee.

While conducting fireboss run on Sycamore #1 belt this morning, member walked through a crosscut at 188 break and stepped in a rut in the bottom. Member heard a loud pop in his left foot and could not put much weight on it. He managed to drive the mantrip outside and transport himself to the Raleigh General.

Absorption of radiations, caustics, toxic and noxious substances

All members plastered stoppings during the move and EE complained after the move of his hands being irritated by the b-bond. It had worn a hole in his gloves and gotten on his bare hands.

Over-exertion in lifting objects

Member was lifting come alongs above head to put into monkey face in order to bring a wrecked flat car over and onto track rail. While lifting he experience a ""pop"" sensation in back, he immediately ceased to work. He came outside and reported injury.

Over-exertion in pulling or pushing objects

Member was bending and pushing bolt into top and heard his shoulder make a loud pop. He did not cease work but told his supervisor about the injury but didn't fill out an accident report. After the shift, he transported himself to Med Express in Beckley. Marfork Safety Department was not made aware of this until after he had seen a doctor.

While Employee was lifting a roll of lifeline he slipped in mud and strained his lower back. He was treated at Raleigh General Hospital.

Caught in, under or between a moving and a stationary object

As Employee was adjusting forks on an underground forklift the fork slid catching his right hand and pinching it between the forks. Employee received five (5) sutures at Raleigh General Hospital.

Caught in, under or between a moving and a stationary object

Employee was attempting to remove a rock that was caught between an impact roller and flow thru. Employee was using a slate bar to remove the rock when the slate bar struck the belt pulling his hand against flow thru causing a laceration to his left finger. He was treated at Raleigh General Hospital where he received five (5) sutures.

While employee was carrying dust filters bent over his hard hat struck the corner of a metal strap which was hanging approximately 6"" from the top causing him to fall on his backside. Employee was complaining of pain in his neck and back. He was transported by EMS to Raleigh General Hospital for treatment. A CT scan was taken and the results were negative.

Employee was bolting in 6 right when a piece of rock approximately 24"" x 3"" x 2"" fell from the rib striking employee on the lower back. Employee was treated by EMT's at the scene and transported by Whitesville Ambulance to CAMC General Division. Employee had xrays taken which was negative but was referred to a spine specialist for further treatment.

Over-exertion in lifting objects

As employee was moving the rub rail on a 14CM15 Continuous Miner he felt a pulling sensation in the lower left abdominal area. Employee was diagnosed at Med Express with a hernia on 5-17-11. Employee returned to work on restricted duty from 5-18 to 5-24. The hernia was repaired on 7-27-11 which the Safety Department was not notified of his surgery until 8-3-11.

The employee was moving a spreader pipe out of a coal screen and caught his right middle finger in a pinch point between the spreader pipe and the screen. Infection has now become present in the wound so EE's physician requested he remain off work until infection clears up.

Struck against stationary object

As Employee was walking he stepped on a rock which caused his right ankle to roll. Employee was treated at Med Express and xrays were taken.

Over-exertion in lifting objects

Employee was using a slate bar to lift shield up on shuttle car to reset breakers. As he was lifting he felt a pain in his lower back. Employee was treated at Princeton Community Hospital. Xrays were negative.

Struck by falling object

Employee was running the motor to section when the canopy roofed out. When the canopy came down it struck Employee on the right elbow. Employee was treated at Raleigh General Hospital. Xrays were negative.

As Employee was carrying a roll of curtain he tripped on a rock causing the curtain to strike the back of his neck. Employee was treated at Raleigh General Hospital. Xrays were negative but was instructed to be on restricted duty for 10 days.

As Employee was scaling top in #5R a piece of draw rock fell out between a row of bolts striking Employee on the head and neck. Employee was treated at Raleigh General Hospital.

As employee was moving continuous miner to the intersection of 5 and 6 to make repairs a piece of rock fell out between a row of bolts striking employee on the left arm. Employee was treated at the scene by EMTs and transported to Raleigh General Hospital for further evaluation. Xrays were taken and was negative but was instructed to be on restricted duty for four (4) days.

While walking around Feeder Employee twisted his ankle on loose gob. Employee was treated at Raleigh General Hospital.

Over-exertion in pulling or pushing objects

While Employee was pulling miner cable out of roadway in the #2 entry he felt extreme pain in his lower back and left upper thigh. Employee went to MedExpress for treatment on Aug 10, 2011 and was put on modified duty due to his injury.

As Employee was supplying bolt machine in #5 crosscut a piece of rock fell striking employee on his left arm causing a laceration. Employee was transported to Appalachian Regional Hospital by EMS for treatment and received nine (9) sutures.

Employee was building a stopping approximately 7 1/2 feet high. Employee stood on a cinder block which overturned causing him to stumble twisting his left knee. Employee was taken to Raleigh General Hospital and was diagnosed with a knee strain.

Over-exertion in pulling or pushing objects

Employee was pulling cable out of gob. As he bent over to pull cable again he felt his back pop and fell to the ground. He was transported to Raleigh General Hospital where he was diagnosed with muscle spasm in his back.

Struck by falling object

Employee and helper was removing a rock off of the roof bolter. As Employee pulled the rock it fell striking him on the right foot. Employee was transported to Raleigh General Hospital by Whitesville Ambulance. Employee received xrays and CT scan. He was diagnosed with a contusion on the top right foot. Employee was given restricted duty until the swelling went down in his foot.

As Employee tripped over a jack and fell against another jack causing a laceration to his left hand. Employee was treated at Raleigh General Hospital where he received three (3) sutures to his left hand.

Struck by rolling or sliding object

As Employee was pulling rock off miner with a slate bar the slate bar became hung. The rock started to slide off the miner which caused the rock to roll in the opposite direction landing on the operators right foot. The rock was approximately 3'x2'x8"". Employee was treated at Raleigh General Hospital and was diagnosed with a fracture on the tip of his right big toe.

Employee was cutting belt with a utility knife. As Employee was cutting the knife slipped causing a laceration on the right leg above the knee. Employee was transported to Raleigh General Hospital where he received 10 sutures.

As Employee was unplugging mantrip a piece of metal was sticking up causing a laceration on his right index finger. Employee drove to Raleigh General Hospital for treatment and received four sutures.

Employee was bolting in #1 when a piece of rock approximately 6""x4""x1.5"" broke around steel. As Employee spun the steel back out the rock broke striking Employee on the face causing a laceration. Employee was transported to Raleigh General Hospital by EMS where he received 8 sutures.

Employee was in the process of closing out-riggers on a 2001 Kenworth Service Truck when his left finger was pinned between the out-rigger and tool box causing a laceration. Employee was treated at Boone Raleigh Medical Center where xrays were taken and received six (6) sutures.

EMPLOYEE LIFTING SHIELD OF THE HIGHWALL MINER THE SHIELD SLIPPED AND CUT EMPLOYEE MIDDLE FINGER OF HIS LEFT HAND

Employee was helping make a belt splice on the section belt. While cutting the belt the utility knife slipped striking employee on his left forearm. Employee immediately ceased work. He was treated at Raleigh Boone Medical Center where he received three (3) sutures.

Caught in, under or between a moving and a stationary object

EE was installing a spray bar into 4A Feeder skirts. Employee and an electrician were rotating the bar. The electrician lost his grip, causing the bar to flip up, smashing Employees right middle finger into the top of the tunnel. Employee was treated at Raleigh General Hospital and diagnosed with a small fracture in the tip of his right middle finger and received 13 stitches.

EE was walking across the bench when his foot hit a exposed root, causing him to trip and fall resulting in him twisting his right ankle

Struck by flying object

Member was operating the continuous miner when a rock came out of the boom and struck him on the lip. Member immediately ceased work and transported himself to the hospital where he received 4 stitches.

Struck by falling object

Member was reaching for drill steel when a rock 36"" wide and 4"" thick fell on his left forearm and shoulder. It cut his forearm with a 4"" gash. Member received 11 stitches at Raleigh General Hospital.

Member was tightening a plug in the rear end of the Kenworth Mud Truck and the ratchet slipped and hit him in the mouth, causing him to break a tooth. Member immediately ceased work to go have his tooth repaired by a dentist.

Member was putting a pump on the 488 scoop. The relief valve was tied back and it came loose and smashed his finger between the pump and relief valve. Member immediately ceased work to go to Whitesville Clinic and had 3 stitches put in his left hand, ring finger.

Member was walking toward the belt line and tripped over a water line buried in mud and hit his tooth on a wing nut on top of structure and broke one of his front teeth.

Member was putting bolt into hole when rock fell and caused bolt to hit him in the private area. He immediately ceased work to receive medical attention.
