A mine examiner discovered a roof fall along the No. 6 Belt Conveyor at 15 Break between the No. 1 and No. 3 entries. The fall measured approximately 100 feet long x 20 feet wide x 7.5 feet high. There were no injuries and no equipment was involved. Passage of work crews was not affected.

Roof fall was discovered by weekly airway examiner at spad #2931.
Miner was assisting in changing conveyor boom on a Joy 14/15 continuous miner when the boom unexpectedly shifted. The miner was caught between the boom and the mine roof injuring miners back. Injury resulted in paralysis below miners waist.
Miner tripped on gravel and a cable and fell into a piece of tin lacerating face and neck. Received stitches.
Employee was assisting the underground forklift operator with the placement of supplies. Machine ran over employees right foot causing bruising and soreness.
Employee was beating two hammers together in an effort to remove a splice pin from a mechanical conveyor belt splice. A piece of metal chipped off one of the hammers striking employee in cheek below the right eye.
Miner slipped and fell twisting knee and resulting in bruising of leg and arm.

While operating scoop, a flyboard bolted to the mine roof, was caught by the scoop and pulled down and fell into the center section of the scoop. A hydraulic fitting was damaged and hydraulic fluid was sprayed into the miner's face and eyes.
While carrying a can of hydraulic oil, miner stepped in a hole in the mine floor causing the miner to twist EE's back.
While backing the roof bolting machine down the no. 3 entry the cable reel was taking up cable causing the trailing cable that was hung to be jerked down striking the miner on the head.

A scoop, 1 break outby, unexpectedly caught the roof bolter trailing cable pulling the trailing cable and entangling the injured's foot in a loop of the trailing cable slack. Resulted in a fractured tibia of the left ankle/foot area.
Employee was welding from an aerial lift when the basket bounced from movement and caught their hand between the handrail and adjacent piece of structure.
While raising a canopy on a JOY 10 S/C Shuttle Car the canopy slipped smashing employee's left thumb between the canopy and the frame of the shuttle car. Employee received 7 stitches. A hairline fracture was also found.
Miner was changing tire on four wheeler when the jack suddenly fell over while under load smashing miner's hand between frame of machine and jack. Resulted in sutures.
While traveling in a rail mounted mantrip, miner injured head when the mantrip struck a broken rail causing the machine to come to a sudden stop. Injury was a laceration requiring 6 staples.
Employee was raising low boy trailer deck by using the hydraulic levers located near the cab end of the trailer. A chain was hooked to the gooseneck of the trailer to secure it, the chain came under tension while adjusting deck causing clevis to break and struck employee in knee. Operator was notified of PT on 07/17/2019.
Miner was working to clear an obstruction on a wash down hose. The line was pressurized causing the end of the hose to fly up striking the miner in left eye. Eye drops were prescribed.
Miner was changing off side top roller on outside stacker belt when EE pinched finger between the roller and the frame of the structure.
Miner was riding elevator from 3rd floor to 2nd floor. Miner reported that the elevator slipped approximately 4 feet causing miner to fall fracturing left radius.
Miner was putting belts on the #9 Slurry Pump. While rolling the belt onto the sheave EE's right pinky finger became caught between the sheave and the belt. Received seven stitches to the end of the pinky finger.
While conducting pre-shift on D10 dozer, miner slipped rolling right ankle. Accident resulted in fractured ankle.
While working on the Raw Coal Classifying Pump, miner was holding key steel for the bushing when EE's finger was hit by another miner attempting to use a hammer to drive key steel in.
