Accident type, without injuries

A fall of roof occured int he intersection of #27 crosscut, #1 entry of the west mains section. Fall was 6 ft high, 12 ft long x 14 ft wide. Roof rock was laminated with clay and mud seams with water present. Roof had been bolted to plan using 6 ft tension/rebar fully grouted oblts. Screen installed and ribs bolted. No injuries or equipment involved. Fall area was rebolted.

Accident type, without injuries

A fall of roof 6 ft high, 16 ft wide by 20 ft long occurred on West Mains at the #10 Crosscut between #2 and #3 entry. The area had been supported. Strata was laminated with clay seams. Additional support will be set through the affected area. No equipment or property damage. Fall was called in and reported.

Accident type, without injuries

A fall of clay rock occured in the west mains #1 entry. Fall was 20 ft long, 16 ft wide, 13 ft high. Area was bolted with 6 ft double lock bolts and was also spot bolted with 12 ft (2) piece double lock bolts. Fall was outby the working section. no disreption of ventilation, no injuries or entrapment.

Accident type, without injuries

A roof fall of clay rock occured in the west mains belt entry. Fall was 20' long, 17' wide, 18' high. Area was supported to place by left double lock bolts, additioinal support of T-5 channel and 12' (2) piece doulbe lock bolts installed on 4' centers. No injuries, 30' of 60"" belt structure damaged.

Accident type, without injuries

A fall of roof occurred in the #3 entry, inby #16 cross cut on West Mains section. The fall was 16 ft wide, 25 ft long, 6 ft high. Area was supported to plan by 6 ft double lock bolts. No injuries or equipment damage.

Struck by falling object

While building a ventilation wall injured was on a ladder placing block on top of wall when the wall collapsed. Right foot was hurt. Foot was too swollen to see if it was broken or fractured.

Person was installing dywidag slings while operating roof bolter, felt pain in lower back. Accident date - 11/27, doctor visit 11/29 - first day off work.

Struck by falling object

Injured was looking upward, pounding wall when something got in his eye.

Injured was carrying post when he felt back strain on 10/30/2006. He worked throught shift and also next shift. He went to ER on 11/1/2006.

Struck against stationary object

Person was operating shuttle car; the car dropped into a hole in roadway causing operator to strike his head against canopy. Person went to doctor and released for work on 9/22/06. He worked 9/26 & 9/27. On 9/28 he reported off work, returned to doctor and is now off work pending diagnosis and treatment.

Person was attaching rail ties to rail. While performing this work he felt a twinge in his left knee. That evening after work his left knee began to swell. X-rays negative - Dr. advised to stay off work to rest knee.

Caught in, under or between a moving and a stationary object

Injured was setting posts in muddy conditions when post fell and hand got caught between posts.

Struck against stationary object

Person was walking down belt entry, stepped down of wood bridge on to wood crib block used for step. Crib block rolled causing knee to twist. Person continued to work thru feb 3, 2006. First day off work for injury feb 6, 2006.

Fall to the walkway or working surface

Walking from shower room to locker, slipped and fell. While falling, he reached out with his left arm to catch himself. Injured left shoulder. He was diagnosed with a torn tendon in his left shoulder. He continued to work until April 17, 2006 when he stopped work to have surgery to repair shoulder injury.

Caught in, under or between two or more moving objects

Injured was unloading steel beams from scoop bucket when they slid together mashing fingers.

Struck by falling object

Employee suffered sprain & hairline fracture when platework load came down on left foot.

Employee was holding a tag line in strong wind when wind blew steel plate attached to line & put his arm in awkward position, tearing rotator cuff.

Injured was building a ventilation wall. While handing block up the wall fell. Injured received a laceration on left leg.

Person was cleaning pine trees to do surveying work. He accidentally struck his face with one of the branches he had cutt off. Scratch to right eye requiring prescription medication for treatment.

Struck by flying object

While drilling into a steel beam at the breaker building to install conduit brackets in windy conditions, it is believed that a piece of steel shaving was blown into injured employees eye. It was stated that the injured employee was wearing safety glasses at the time of injury.

Struck by falling object

Employee was relocating acetylene bottle to storage rack and forks unforklift shifted causing but the to slide off forks. He smashed l thumb attempting to stop bottle from sliding.

Fall onto or against objects

Person was going over belt cross-over; his foot slipped on step causing a fall. He landed on his self-rescuer, pushing the rescuer into his lower right ribs. Person reported the accident that day but did not go to a doctor at that time. He continued to work; went to doctor on 4/24/06 and follow-up for x-rays. Company was not informed of doctor's diagnosis until 5/15/06.

Caught in, under or between a moving and a stationary object

A 5' drill steel fell out of the roof and caught the left hand between the steel and steel extension. The operator thought the steel was clamped in the guide jaws but the insert had come out prior to the operator closing the jaws onto the steel. The belt securing the insert into the jaws had come out allowing the insert to come out. Sutures to close wound.

Inhalation of radiations, caustics, toxic and noxious substances

Report filed for occupational illness. Person has worked on continuous miner section during his mining career operating roof bolters and continuous miners.
