A roof fall occurred on # 16 butt section, in the # 2 entry working face. No injuries occurred, and no equipment damage was incurred. The roof and ribs were supported to the roof control plan. The mine roof immediately preceding this fall had supplemental roof support in the form of roof screen,& v-beams & jacks.

An unintentional roof fall occurred and was discovered by mine personnel. No injuries occurred. No equipment damage occurred. The roof fell approx. 6 to 8 feet above the coal seam, 16 feet in width x 30 feet in length. The fall area had been roof bolted and rib bolted to plan. The roof fall will be cleaned up and re-supported with a steel arch system.
An accidental roof fall occurred at # 17 block, West Mains rubber tired haul road, and was discovered by mine personnel. The roof fell approx. 12 to 14 feet above the coal seam, 16 feet in length, and approx. 30 feet long. The fall area had been roof bolted & rib bolted to plan. This roof fall will be cleaned up & re-supported.
Roof fall occurred on the 16-Butt MMU-003 section after deteriorating roof conditions were discovered. A attempt was being made to installed additional roof support but failed to prevent the fall. MSHA Hot Line was immediately contacted and notified. No injuries or equipment damage was reported as a result of this event.
A roof fall was discovered in the 3-Butt, #2 entry, #4 crosscut during the pre shift examination. The fall area was approximately 40-ft long, 16-ft wide and from 4 to 6 ft above anchorage. Fall consisted of laminated and stack rock. The fall was immediately called in and reported to both MSHA and the State.
Top of utility pole caught fire after insulator shorted out. Power was disrupted to the mine causing the evacuation of the mine. When discovered, the incident was called into the Hot Line number. It was later verified that utility pole was not on mine property.
Employee lifted a compressed gas cylinder, started to walk, stumbled/slipped, and strained his right shoulder. Employee finished the work shift, and continued working with no time lost, until January 18th 2013. On January 18 2013 the employee stopped working to acquire a surgery to repair his right shoulder rotator cuff.
Employee was pulling on a mine de-watering pump that was stuck in the mud, causing pain in the employee's abdomen.
Employee was walking along the belt line, when he slipped/tripped and started to fall. Employee reached out and attempted to catch him self by grasping the belt structure side rail, which caused pain in the employee's arm.
Employee was positioned under the ventilation tubing & pushing/sliding the ""slider tube"" out of the ventilation tubing towards the working place, at which time the immediate mine roof rock fell on to the ventilation tubing, knocking the employee on to the ground, causing the injury.
Employee was using a Simplex mechanical jack to re-rail track haulage equipment and over-exerted himself , causing pain to his neck and shoulder.
Employee was attempting to retrieve a chain with hooks off a L/Wall support, when one end of this chain became ""snagged"" on something , at which time the employee ""jerked"" on the end of the chain , which then recoiled and struck the employee's hand/thumb.
Employee stepped off center walk way to the mine floor at the rear end of a roof bolting machine, and injured his left knee.
Employee was helping co-workers physically pull and relocate the power cable for the section ventilation auxiliary fan, when the cable they were pulling on became stuck and came to an abrupt stop, the employee felt pain in his lower abdomen area.
Employee was walking on under/ground road way , slipped in rut in road way , which was wet and muddy.
Employee was a passenger in a rubber tired man trip on the way out of the mine, when the front bumper of the man trip contacted the end of the track haulage rail and stopped abruptly, causing the employee to be forced backwards into the passengers seat.
Employee was attempting to move a ABS 50 de-watering pump and as he was doing so felt a pain in his lower back. Employee completed his shift and later sought medical attention. Employee is now off work for stain/sprain of lower back muscles.
Employee was changing cutting bits on the shear head gate drum. While doing so a section of the face slide down and part of it landed on his left foot. The employee was examined at the local ER and given a full release to return to work. Employee continued to have foot pain and sought medical attention and is off work as of 07/16/2012 with a deep contusion injury to the left foot.
Employee was loading 20-inch ventilation tubing into a scoop bucket and afterwards felt a sharp pain in his upper back and left shoulder.
Employee was installing plug on welding lead and was using a screw driver to tighten leads. The screw driver slipped and punctured left hand between index and middle finger. Employee continued to work but sought medical attention after his shift when his hand began to swell. Employee is off work being treated for infection.
Employee was using a 6-ft step ladder to reach the top of a stopping to apply b-bond. As he was stepping from the 3rd to 4th rung of the ladder, the ladder twisted and started to fall. The employee jumped from the ladder and when he came down onto the mine floor he twisted his left knee.
Employee was assisting other miners hang the continuous miner cable across the intersection and when doing so he felt a pain in his right shoulder. Employee completed his shift and sought medical attention afterwards. Employee was not released to return to work by his doctor.
Employee was putting hydraulic oil into the roof bolter and had checked the fluid level in the site glass. When the employee went to move, his right foot stuck in the fireclay bottom causing his knee to twist injuring right knee
Employee was tramming shuttle car from feeder to the continuous miner when he hit a bump in the roadway. The resulting jolt caused pain in his back. Employee sought medical treatment and was not released to return for his next scheduled shift. First day off work 03/30/2012
Employee was installing rib screen in the teardown room and when he raised a section of screen to the rib he felt a pain in left shoulder. Employee continued to work but pain worsened. Sought medical treatment for condition on 03/26/12 and was not able to return to work.
Employee was helping build cribs in the mine and sprained his ankle. Report submitted to terminate citation # 7040529
Employee was taking welding leads through line curtain and stumbled over 30-inch crib block placed there to hold curtain down. In an attempt to regain his balance he twisted his right knee causing injury.
Employee was in the process of reinstalling the relay bar retaining pin that had worked loose on shield #121. The pin became wedged and as it was being worked back and forth to free up the employees left thumb was pinched between the pin and the relay bar avulsing the tip of the thumb.
Crew was installing additional 4"" line to tie into the existing 6"" air line, when they were unable to loosen the 2"" and the 3"" coupler from the ball reducer they started to remove the 6"" coupler. As the second bolt was loosened, the coupler blew off striking the employee in the left knee.
Employee was stacking wooden cribs by hand to construct a ""spill board"" at a belt line discharge point, when he struck/mashed his left index finger between a crib block and the discharge units steel frame.
Employee was riding in a Diesel mantrip exiting the Mine when the mantrip operator attempted to avoid some irregularities in the road way, and bounced into some tire ruts, causing the mantrip passengers to be jarred around.
Employee stumbled, slipped, and fell while walking in the coal mine, injuring his knee. Report submitted to terminate citation # 7040531
Employee was installing tensionable sling system in the roof and as he was installing the connecting hardware in place the bolter canopy was inadvertently raised pinching the employees left index finger between the roof and canopy.
Employee was working in the belt entry and while doing so walked into the 8-inch fresh waterline. Employee had neck pain afterwards and was treated by a chiropractor.
Employee was cleaning up section tool car and pinched the middle finger of left hand while handling continuous miner cat pads.
