An area of broken up slate stone approx. 10 feet in width and 8 feet in length and approx. 7 feet high fell. This area was bolted according to the plan. This fall was cleaned up and the mine roof was re-supported. There were no injuries, equipment, or entrapment. This fall was immediately reported to MSHA. MSHA representative was on site and inspected this roof fall.

A roof fall was discovered at 10:40 a.m. on 08/10/10 by a fireboss. Location Westmains 49 BK to 4 CC. The fall was 14' wide, 32' long and 6' high. The fall will not be cleaned up. Both entrances to the fall have been cribbed off.
A rib rolled occurred at the intersection of West main #2 entry and #64 crosscut impeding passage in the alternated escape way. The section of rib measured 15'long X 5' high and 12' deep. The area had been rib bolted to plan with additional floor to roof support set. Immediate notification was made at the time the condition was discovered.
Employee was standing on the pontoons of the #2 longwall face support when a rock fell from in between the #2 and #3 longwall face supports at the tip. The rock struck the top of the headgate gearbox cover and when doing so it bounced off the cover and then struck the employee on the left foot causing injury.
Employee stepped between two longwall supports to allow another miner to walk passed. When the employee stepped back out from between the supports he lost his balance and fell down. His right foot was still between the supports pontoons when falling causing injury to his right ankle.
Employee was advancing panline when a piece of rock fell from two supports and struck top of right hand. Employee was examined/x-rays and was told it was only bruised and released to return to work. On 10/23/2010 the employee was re-examined/x-rays and was told he had a fractured bone in his right hand. Hand was splinted and he was released to return to work.
EE was using a 3 ton chain come-a-long to gather slack chain being generated by the headgate tensioner when the face conveyor flight that was being held in place by the stops broke. This let tension release back into the face conveyor chain, causing the hook on the chain come-a-long to bend & release striking EE on the left lower leg causing injury.
Employee and another assistant were using a slate bar to carry a dolly of the push/pull unit from the belt entry to the intake entry. While walking backwards, the employee stepped onto a bundle of wedges causing him to stumble. The weight of the dolly shifted on the slate bar towards the employee injuring the employee's back.
Was shoveling along reclaim belt and was attempting to put material onto belt when he felt slight pull in stomach/groin area. After completing shift noticed a slight bulge in belly button.
EE was in the process of performing maint. adjustments on a roof bolting machine and while using a prybar the EE was struck by the bar, causing a laceration to the EE's right ear.
Employee was installing hardware for roof slings when the socket came off of the hydraulic wrench he was using to tighten components. The socket struck the employee above the left eye causing a laceration requiring sutures to close.
Employee was using a sledge hammer to drive a wood wedge under a locomotive while re-railing. When doing so the ring finger of his left hand was mashed between the hammer handle and the steel step of the locomotive. Injury required sutures as treatment.
Operator was notified on 12/02/2010 that the employee had participated in a medical examination authorized by the Mine Act of 1977 and examination indicated that the miner has a sufficient degree of pneumoconiosis. This employee is exercising this rights under Title 30, Part 90.
