Accident type, without injuries

An unintentional roof fall occurred and was discovered by mine personnel. No injuries or equipment damage occurred. Fall area measured 6 feet highx 16 feet long x 16 feet wide. This fall area had previously been roof and rib bolted to plan , with roof screen installed. This fall material consisted of laminated shale with a clay seam parting , with water present.

A battery powered personnel carrier was parked U/G and later discovered on fire, requiring employees working in/by to don SCSR's and evacuate out/by to a safe location.

Unintentional roof fall occurred and was discovered by mine personnel. No injuries or equipment damage occurred. The fall area measures 16 feet wide x 30 feet long x 7 to 8 feet high, above the coal seam. The fall area had previously been roof & rib bolted to plan. This fall area has been ""dangered off"" with ""breaker posts"", set on both accessible sides of the fall.

Fall onto or against objects

Employee was assisting to ballast the track rail and crossed the parked equipment between the ballast cars, getting his feet tangled in the hydraulic hoses which operate the ballast cars. Employee then slipped/tripped and started to fall, reached out to catch himself and over exerted his hand and wrist.

Fall to the walkway or working surface

Employee was walking in the belt heading when he slipped on the mine floor and landed on his left knee and jammed his left hip, causing pain to his knee and hip.

Over-exertion in lifting objects

Employee was carrying concrete blocks to assist in the construction of a stopping wall when he felt a sharp pain in his wrist. Pain became severe and the employee was transported to the local hospital, where he was diagnosed with a fractured wrist.

Struck against a moving object

Employee was operating a Scoop along the intake haul road, when the machine encountered a road way irregularity, which jarred the employee and caused him to strike his head on the machine canopy.

Employee was operating a scoop in the track entry, and while traveling out/by the machine contacted the terminated end of the track rail, causing an unexpected abrupt stop of the machine, and caused the employee to strike his head on the cab of the operators deck.

Caught in, under or between a moving and a stationary object

Employee was retrieving a mine de-watering pump, when the rib sloughed/rolled out and struck the employees hand against the pump, causing injury to his thumb.

Struck by falling object

Employee was assisting in long wall tear down and was retrieving the wedge setting tool when he was struck in the head by a 5 X 5 prop causing him to strike his right knee on the bottom

Fall onto or against objects

Employee was cleaning up rib roll material off the mine floor and took a step backwards , lost his footing and fell onto/against the rib roll material, causing pain to his left arm

Over-exertion in lifting objects

Employee bent over and picked up a ""red devil"" come-a-long hoist, and incurred pain in his back

Employee was assisting in the retrieval of track rail. When the Scoop was pulling the left rail, the end of the right rail caught on the end of the right out/by rail, causing the piece that was being pulled to deflect into the cross cut, striking the injured employee.

Employee was helping make a belt splice and using a power rivit driver installation tool, and got smoke/residue in his eye. Report submitted to terminate citation # 7040533

Employee was stepping down from an elevated position and rolled/twisted his ankle. Report submitted to terminate citation # 7040534

Employee opened a hot radiator cap on a Diesel machine, and got sprayed with engine coolant. Report submitted to terminate citation # 7040535

Employee was using pliers to pull a nail out of a roll of belting, when he lost control of the pliers and struck himself in the mouth. Report submitted to terminate citation # 7040530

Struck by falling object

Employee was increasing an air regulator opening in a brattice wall by knocking additional concrete blocks out, when an unnoticed loose block fell out, striking the employees thumb.

Employee was attempting to move wooden crib blocks by swinging/striking/sticking the pointed end of a pick hammer in the end of the wooden blocks, when the hammer glanced off the edge of the wood and struck the employee in the foot.

Caught in, under or between a moving and a stationary object

Two employees were installing supplemental roof support, and when one of the employees moved a post to reposition it to be cut to length, the other employee tried to help, and got his hand caught between the end of the post and the coal rib, causing injury to his fingers.

Over-exertion in lifting objects

Employee was unloading supplies off a rail mounted supply car and felt pain in his wrist. Report submitted to terminate citation # 7040532
