Accident type, without injuries

Roof fall was discovered on E-mains #6 entry @ #9 cross-cut spad #80. Outby section and loading points. Developed Sept. 2005. 190' coverage. Area bolted with 6' double lock bolts, 7/8"" dia. grade 75, 8x8 inch roof plates with roof screen installed. Fall measured 25' long, 20' wide x 9' high. No injuries or equipment involved. No ventilation or escapeway blockage.

Accident type, without injuries

Roof fall discovered on the North Mains, outby section & loading points. Developement date: Nov. 2006, 300 ft. coverage. Area bolted with 6 ft double lock bolts - 7/8"" dia, grade 75, 8x8 inch roof plates with roof mat installed. Fall: 16 ft wide x 16 ft long x 10 ft high. No injuries, no equip involved, no ventilation or escapeway blockage. Notification made within 15 minutes.

Accident type, without injuries

Roof fall occurred in the #9 cross-cut of E-Mains between #4 and #5 entries. Fall was 55 ft long, 16 ft wide and between 12 and 15 ft in height. No injuries or equipment damage. Fall did not affect intake iar courses or any escapeways. Additional roof support was installed at both ends of fall area to protect surrounding area. Notifications were made within 15 minutes.

Accident type, without injuries

A roof fall was discovered on west main at #13 cross-cut between #2 and #3 entries. Area of the fall was inaccessible because of additional roof support that had been set in the entries prior to the fall. No injures, no equipment damage. MSHA and wv hot lines notifications were made at the time of discovery.

Roof fall occurred & found on wet mains, #4 entry, #16 crosscut. Fall material made up of laminated rock & clay & clay vein. 9' H x 20' L x 25'W. Fell in crosscut extending into the entry. No injuries & no equipment damage. Immediate notification to MSHA and WV hot line numbers.

A roof fall was discovered on this date 1/23/07 in the West Mains # 5 Entry between #11 and #12 Cross-Cuts. Fall was discovered by the weekly examiner and reported immediately. The MSHA call center & District #3 was notified at the time found. Could not determine exact date or time of fall. No Equip damage or injury. No disruption of ventilation.

Bodily reaction, (Not Elsewhere Classified)

EE was walking in warehouse when he felt a pain in his left lower leg. He did not slip/trip or fall. He did not step on any object or materials. No incident or accident occurred. Walking on cement floor. He went to his dr on 12/11/07 and has been off work since that date.

Over-exertion (Not Elsewhere Classified)

While operating the longwall shear, EE stepped down from a relay bar onto the mine floor and injured his lower back. The step down was approx 10-12"". EE finished shift and returned to work for his next shift, but left early because of back pain. Went to DR on 11/30, X-Ray and Rx, returned to work on 12/3. Started missing work on 12/4 for injury.

While walking to get to shuttle car, ee slipped on pavement twisting his right knee. Person finished his shift, went to doctor after work. First day off work for injury 11/6/07.

Person was walking out of tool room of shop and slipped on concrete floor. person did not fall but twisted lower back. Floor was not wet and no obstruction. First day off work due to injury 10/22/07.

Over-exertion in pulling or pushing objects

Person was pulling hoses and light cable from between longwall shields to prevent damage. He felt pain in his left shoulder. He was taken to E.R. and examined, diagnosed with a strained muscle in left shoulder - placed arm in sling and told not to return to work for one week.

Struck against stationary object

Person was walking in West Mains belt entry performing pre-shift examination, when he stepped on a rock, twisting his right knee. Worked remainder of shift and 7/31/07. Went to doctor on 8/1/07, 1st day off work. Swollen ligament inside of knee and fluid behind knee cap.

Over-exertion in lifting objects

Person felt pain in right shoulder while lifting a duke (water prop). He contined to work. Went to DR on 7/16/07. First day off work on 7/17/07 for torn rotator cuff in right shoulder.

Fall from machine

Person was getting out of Jeffrey locomotive when he slipped on step causing to fall injuring, right shoulder and left lower leg. Continued to work. Notified Mettiki on 7/9/07 he had gone to dr. First day off work was 7/12/07 for torn ligament in lower left leg.

Fall from ladders

Injured was on a ladder building wall when a highline cable fell and got caught on motor and knocked over ladder injuring his lower left arm.

Person was making exam of permissible pump, stepped on 3"" plastic pipe with right foot while walking and slipped.

Absorption of radiations, caustics, toxic and noxious substances

Injured's eyes and arms were red from chemical reaction.

Feet got stuck in mud while carrying posts. He hurt back.

Fall to the walkway or working surface

Slip on ice in parking lot & broke left leg.

Injured was installing chain on scoop bucket. He stepped off and twisted right ankle.

Employee was striking two hammers together while setting posts when one of the hammers chipped off and the metal imbedded into the index finger of his right hand.

Fall onto or against objects

While walking on jobsite injured stepped on a piece of sheet metal siding that was covered with snow. Employee slipped, landing on his back and striking his shoulder against some cribbing blocks.

While aiding 4 other co-workers move a hydraulic skid, top portion slid off of base pinching injured employees right thumb.

EE was pulling winch cable from a winch mounted on a DBT scoop. The cable became tangled and his right index finger was pinched between the cable and winch as he was trying to untangle it. His right index was lacerated requiring sutures to close.

Person was stepping up onto Knack tool box to hang a cable. The tool box kicked off causing him to fall. When he fell his head struck the tool box resulting in lacerations to his upper and lower lips that required sutures to close.

Person was chipping off slag from bit lug he had welded. A piece of slag went under his safety glasses and into his right eye. Person was examined by doctor - abrasions to right eye and given prescription eye drops. No time missed from work.

Over-exertion in pulling or pushing objects

Person was attempting to remove a wood board from under forklift forks. Pulling and twisting caused back pain. Person went to doctor - x-rays, injection and prescription.

Prying rubber seal on junction box with a 4"" screwdriver. Hand slipped causing screwdriver to come in contact with right cheek of face that required sutures to close.

Over-exertion in lifting objects

Person was lifting oxygen bottle from scoop bucket & felt pain in lower back. Went to Dr. 5/9/07 was treated & released. No lost time, rec'd chiropractic treatment for injury. Medical treatment only.

Struck by falling object

Person was sitting at the section power car when a piece of rock fell from the roof and struck him on the top of left upper leg. Rock fell from between the rib and installed roof mat.

Injured employee was attempting to free a piece of 3/4"" PVC conduit that was frozen to the groud. Injured employee bent conduit to break, conduit snapped and struck injured employee in the right index finger causing a laceration.

Person was getting into shuttle car & door closed on left hand causing inj to little finger. Person was seen by dr on day of accident but was not released pending appt with orthorpedic surgeon. Was seen & released 1/18. Person was off waiting on appt not off for injury. Could have returned to full duty 1/16.

Notification was received on 2/13/07 that ee was found to have a standard threshold shift for both ears. Thsi was found during his annual hearing test.
